     【摘要】有研究指出:伽玛刀用于治疗三叉神经痛,不但可以降低外科手术治疗这种疾病带来的危险性,还可以减少对感觉神经的损害。【全文】Gamma Knife Radiosurgery Effective In Trigeminal NeuralgiaBy Maggie Schwarz Special to DG News WASHINGTON, DC -- March 27, 2001 -- Gamma knife radiosurgery is a minimally invasive, safe and effective treatment for trigeminal neuralgia, researchers report. This technique does not carry the risks of many other surgical approaches used to treat this condition and offers a lower risk of neurosensory functional loss, according to Steven J. Scrivani, MD, DDS, Columbia University, New York, New York. Dr. Scrivani presented these findings at the 26th Annual Scientific Meeting on Orofacial Pain and Temporomandibular Disorders, in Washington, DC. The prospective pilot study enrolled 81 patients who continued to have trigeminal neuralgia despite medical and/or surgical treatment. Twelve had multiple sclerosis and one had a benign tumor. All patients underwent gamma knife radiosurgery. A maximal radiosurgical dose of 70 Gy was administered to the isocenter, with the brainstem surface irradiated with no more than 20 to 30 percent isodose. Pain relief, latency interval to pain relief, need for further treatment and complications were evaluated. Follow-up ranged from three to 16 months. Seventy-two patients (88 percent) had excellent or good results with complete relief or significant improvement in pain. Four patients (5 percent) did not achieve pain relief and were subsequently managed medically. Recurrence of pain was treated with a second gamma knife radiosurgery in four patients (5 percent), three of whom had multiple sclerosis. The latency interval to achieve pain relief ranged from one day to three months. Paresthesia or dysesthesia occurred in eight patients (10 percent). No patients experienced anesthesia. There were no short-term complications, nor any long-term side effects. Gamma knife radiosurgery appears to be offer a minimally invasive, safe and effective approach to management of trigeminal neuralgia, Dr. Scrivani concluded.
