生物工程皮肤治糖尿病足溃疡

    Bioengineered Skin Heals Diabetic Foot Ulcers

    NEW YORK (Reuters Health) - An experimental, bioengineered tissue grown from human skin appears to speed healing of diabetic foot ulcers, according to results of a study.

    Due to poor circulation and a reduced ability to fight infection, many people with diabetes develop foot ulcers that are difficult to heal. Sometimes the damage can be so severe that a foot has to be amputated.

    Currently, treatment involves dressing a wound to keep it moist, but an experimental type of bioengineered skin tissue called Dermagraft appears to provide swifter and more extensive healing, according to Dr. Jason Hanft, a podiatrist at the Foot and Ankle Institute of South Florida in South Miami.

    In an interview with Reuters Health, Hanft explained that skin cells from infant foreskins are grown on an absorbent mesh to form the tissue. The cells from one foreskin can eventually grow to form 250,000-square feet of skin cells, he said.

    Once the tissue is formed, it is frozen until needed. After being placed in hot water to thaw, the tissue is applied to the ulcer and covered with a nonstick bandage and a moist dressing. Hanft noted that the tissue is not meant to replace skin. Instead, it induces and improves the body‘s natural healing abilities and provides a ``scaffold‘‘ for skin cells.

    In the current study, 28 patients with diabetic foot ulcers were randomly assigned to have their wound treated with Dermagraft or with the gauze and bandages alone. Patients received as many as eight applications of the tissue during the 12-week study. Both groups of patients received special inserts to wear in their shoes and had follow-up visits once a week.

    After 12 weeks, about 71% of chronic ulcers had healed in patients treated with the tissue compared with just 14% of those who only received gauze, according to Hanft. On average, the percentage of wound area that healed was twice as high in wounds treated with the bioengineered tissue, he noted.

    In addition, the wounds treated with the tissue healed significantly faster than those covered with gauze, and infections occurred less frequently in patients treated with the tissue, the researchers report.

    Hanft reported the findings this week in New Orleans, Louisiana, at the annual meeting of the American College of Foot and Ankle Surgeons His research is part of a nationwide trial of Dermagraft.
