    肺肉瘤样癌14 例临床分析

    【摘要】 背景与目的 肉瘤样癌(sarcomatoid carcinoma ,SC)是一种较少见的癌和肉瘤样形态混合于一

    个瘤体内的恶性肿瘤。本观察拟探讨肺肉瘤样癌的临床、 病理特点。方法 回顾性分析 14 例经组织病理学

    证实的肺肉瘤样癌的临床表现、 病理特点、 治疗经过并进行随访。结果 肺肉瘤样癌发病平均年龄为 62 岁 ,男女之比为6 比1 ,临床表现与其他类型肺癌相似。其增强 CT有一定特征性 ,但纤维支气管镜检查难以明确

    诊断 ,免疫组化均有上皮表型。结论 肺肉瘤样癌的诊断主要依靠手术病理 ,不能行手术者可能漏诊 ,免疫组

    化对其诊断及鉴别诊断很有帮助。治疗策略与非小细胞肺癌相似。

    【Abstract】 B ackground and objective Sarcomatoid carcinoma (SC) is a rare malignant cancer with mixed

    tumor and sarcomatoid tissues. The aim of this study is to investigate the clinical manifestations and pathologi2

    cal findings of sarcomatoid carcinoma of the lung. Methods Data including clinical manifestations , pathologi2

    cal findings , t reatment were ret rospectively analysed f rom fourteen patient s with lung sarcomatoid carcinoma

    confirmed by pathology and follow2up was carried out . Results Mean age at onset was 62 years old and gender

    ratios (M/ F) in these patient s was 6 ∶1. The clinical manifestations of lung sarcomatoid carcinoma were simi2

    lar to that of other types of lung cancer while there were characteristic findings on the enhancement CT scan.

    Bronchofiberscopy was not a reliable examination for final diagnosis. Cells with endothelial phenotype could be

    detected by immunohistochemical method. Conclusion The final diagnosis of this disease depends on his2

    topathological observation , while the diagnosis may be missed among patient s without surgical intervention.

    Immunohistochemical examination is helpful for diagnosis and differential diagnosis. The therapeutic st rategy

    is coincident with that for non2small cell lung cancer .

    肉瘤样癌( sarcomatoid carcinoma , SC)是一种较

    少见的癌和肉瘤样形态混合于一个瘤体内的恶性肿

    瘤 ,可以发生在全身许多部位 ,但以上呼吸道、 肺、 乳腺

    和肾常见。本文总结我院从 2000 年至 2005 年收治的

    经过手术后病理学证实的 14 例原发性 SC 患者的临

    床特点,并结合文献对其组织发生、 病理特点、 临床表

    现、 治疗和预后进行讨论 ,以期提高对本病的认识。

    1 临床资料

    1. 1 病例资料 表 1 列出了 14 例患者的临床病理资

    料。其中男性 12 例 ,女性 2 例 ,男女之比为 6 比 1 ,发

    病平均年龄为 62 岁。全组随访 12 例 ,失访 2 例 ,截止

    观察结束时有 5 例死亡 ,7 例仍存活。

    1. 2 起病症状 咳嗽、 咳痰 10 例次 ,胸背痛 7 例次 ,痰中带血 6 例次 ,仅 1 例发热 ,无症状体检发现 1 例。

    1. 3 胸部 CT 表现 肿瘤部位:周围型 7 例 ,其中 4

    例与胸膜关系密切者均伴有较明显胸痛;中央型 7 例 ,其中 3 例合并阻塞性肺不张。CT 检查中 13 例可分

    别见到毛刺、 分叶、 胸膜牵拉或侵犯胸壁等恶性征象 ,影像学考虑诊断为肺癌。1 例 CT 表现为低密度肿

    块 ,密度均匀分界清楚 ,影像学诊断考虑为肺囊性变合

    并感染。

    1. 4 纤维支气管镜检查 14 例中 11 例术前行纤支

    镜检查 ,8 例有异常发现 ,其中 7 例可见新生物或粘膜
