5、老年人疝术后并发症的防治
5.1老年人疝术后一般并发症
Gunnarsson[2]对比发现老年人疝术后除肺炎外其它心脑血管并发症无明显区别。因此，早期活动，鼓励咳嗽排痰，注意营养及水电解质平衡，控制感染，是预防肺部和心脑血管系统并发症的关键。
5.2老年人疝术后应特别重视的并发症
(1)切口感染，老年人免疫功能低下，加之腹股沟区卫生欠佳，切口感染率增高。有学者报告[14]切口感染率3.5%，窦道形成率0.13%～0.60%。作者[15]在援非医疗队见到2例老年腹股沟疝，术后切口感染窦道形成而并发破伤风，导致严重后果。因此，术前皮肤准备、术中认真止血及补片妥善固定，必要时行负压吸引，是预防感染的重要环节。(2)尿潴留是疝术后常见并发症，老年人人高达30%[14]。常规留置导尿和积极治疗前列腺肥大是防治术后尿潴留和尿路感染及术后近期疝复发的重要措施。(3)下肢深静脉血栓形成，老年人高达15%[14]。我院曾有1例疝术后发生深静脉血栓导致肺栓塞猝死。术后早期活动是预防血栓形成的重要措施。(4)腹股沟区持续疼痛，可因术中损伤髂腹下神经或髂腹股沟神经所致。也可因McVay法修补深及耻骨骨膜引起骨膜炎所致。因此，术中仔细解剖避免神经损伤和骨膜损伤是重要的预防方法。术后局部理疗和封闭疗法可缓解症状。
