循证医学与呼吸疾病学(3)
前些年，为探讨治疗COPD患者肺动脉高压，曾经花费了不少人力、物力、财力，尝试使用各种扩血管药物来降低肺动脉压力。虽然，某些研究结果显示，有些药物的确可以降低肺血管阻力和肺动脉压力，但是有许多问题尚有待进一步研究，诸如这些扩血管药物对血管作用缺乏选择性，在扩张肺血管的同时也会引起全身血管扩张，因而会降低体循环血压和心排血量，因此需要全面评价和权衡其对机体的整体作用。其次，这些药物只能暂时地降低肺血管阻力，不能从根本上逆转肺动脉高压，故其远期疗效很不肯定，因而这些药物对于患者自然病程、有效寿命以及生存率、病死率的影响就更需要研究了。因此，必须在循证医学的指导下，进行大量的、全面的、对照双盲、多中心的研究，对这个问题才能有一定的了解，才能对临床有指导意义。
循证医学是临床医学发展的必然。呼吸专业同样应该在循证医学的指导下发展与进步。与其他学科相比，循证医学在呼吸科发展较慢的问题，可能会影响呼吸专业的发展。因此，应该向呼吸专业的同仁呼吁，大家都来重视循证医学，用循证医学的基本观点来指导临床实践，就会加快呼吸专业发展与进步。
