血清肌钙蛋白I升高与病毒性心肌炎诊断及病程关系探讨(3)
原发性扩张型心肌病患者血清中出现抗β肾上腺素能受体、抗胆碱能M受体抗体等,自身抗体发挥受体激动剂效应,参与了发病和进展[10]。本研究对cTnI持续升高的VMC患者25例以及病毒扩增阳性、cTnI升高的15例进行观察,血清内出现抗M受体抗体,减少心肌M2数目,降低亲和力,即导致M受体下调和去敏(表1)。cTnI持续升高、CVB阳性的VMC血清与扩张型心肌病患者血清内抗M受体抗体对心肌M2受体下调和去敏作用相似。由此不难看出,cTnI持续升高的患者,至少其中一部分自身免疫紊乱、损伤心肌M受体,可能已开始VMC向扩张型心肌病的进程。mtDNA转录和编译氧化磷酸化酶调节细胞内的能量产生和利用,mtDNA特别易受损伤而发生缺失和变异。本研究提示,VMC淋巴细胞mtDNA缺失发生率是对照组的15.8倍,VMC患者心肌细胞mtDNA缺失发生率比正常人高2.5倍 (表2)。cTnI持续升高的VMC患者mtDNA改变与向扩张型心肌病发展的关系尚待进一步探讨。
