    Protective effect of Shenfu injection(参附注射液) on cardiac function in patients undergoing Fallot's tetrad radical operation under cardio-pulmonary bypass

    Abstract: Objective: To investigate whether Shenfu injection(参附注射液) has protective effect on cardiac function in patients undergoing Fallot's tetrad radical operation under cardio-pulmonary bypass. Methods: Ninety-six patients undergoing Fallot's tetrad radical operation were randomly divided into Shenfu injection (SF) group and control group. The SF group was treated with 1 ml/kg Shenfu injection intravenous at 30 minutes before anesthesia, the control group was infused with 1 ml/kg saline intravenously. The changes of hemodynamics and pulse oxygen saturation (SpO2) during surgery, dose of vaso-active drugs intra-and postoperatively, recovery time during postoperative period were observed. Results: In SF group, mean artery pressure (MAP) and SpO2 were higher and heart rate(HR), central venous pressure(CVP) were lower (all P<0.05), the doses of dopamine, dobutamine, sodium nitroprusside and cedilanid were lower intra-and postoperatively (all P<0.05), and the extubation time and ICU stay time were shorter during postoperative period (all P<0.05) than those in control group. Conclusion: Shenfu injection has protective effect on cardiac function in patients undergoing Fallot's tetrad radical operation.

    Study of matrix metalloproteinase 9 expression after mice ischemia/reperfusion in hippocampus and treatment effects of Yangyin Tongnao granule (养阴通脑颗粒)

    Abstract: Objective: To investigate the expression of matrix metalloproteinase 9(MMP9) after cerebral ischemia/reperfusion in hippocampus, and to explore the role of MMP9 during cerebral ischemia/reperfusion and the treatment effect of Yangyin Tongnao granule (养阴通脑颗粒) on the brain injury induced by cerebral ischemia/reperfusion. Methods: Immunohistochemistry was used to measure the protein expression of MMP9 after cerebral ischemia/reperfusion. Results: MMP9 expression was significantly increased at 20 minutes after ischemia/reperfusion in CA1 in model group〔(42.3±17.1)positive cells〕 compared with sham-operated group〔(4.8±1.6)positive cells〕, P<0.05. In drug-intervented group, the MMP9 expression was lower in Yangyin Tongnao granule which was 〔(7.2±2.9)positive cells〕 and nimodipine group which was 〔(12.7±5.3)positive cells〕 than those in model group, all P<0.05. Conclusion: The results demonstrate that the increase of MMP9 expression in CA1 could be induced by cerebral ischemia/reperfusion and MMP9 may be one of the molecular mechanisms of brain damage. Yangyin Tongnao granules are of the prevention and treatment effect on the brain injury induced by cerebral ischemia/reperfusion.

    Effect of Chinese angelica parenteral solution(当归注射液) on mesenteric lymph microcirculation in blood stasis (血瘀证) rats 

    Abstract: Objective: To study the effect of Chinese angelica(当归) on lymph microcirculation. Methods: Sixteen blood stasis rats were divided randomly into treatment and control groups (n=8 in each group). The treatment group was firstly treated with Chinese angelica parenteral solution (当归注射液,20 mg/kg), after 20 minutes, refusion of withdrawn blood and Chinese angelica parenteral solution was performed secondly. Chinese angelica parenteral solution was replaced by normal saline in control group. Changes of mesenteric lymph microcirculation were observed and recorded by using a video microscope with TV recorder. Results: After the treatment of Chinese angelica, the contracting extent and spontaneous contraction frequency were increased, contracting fraction(IndexⅠ), contracting activity (IndexⅡ) and lymphatic dynamics(L.D-Index) of micro-lymphatics were significantly higher than those of control group (all P<0.05). After the second treatment, the all mentioned indexes except Index Ⅰ were significantly higher than those of control group (all P<0.01). Conclusion: Chinese angelica parenteral solution can increase the contractility and spontaneous contraction frequency of mesenteric micro-lymphatics, improve lymph backflow in blood stasis rats.

    Effect of ligustrazine(川芎嗪) combined with albumin in acute cerebral hemorrhage after microtrauma clearing

    Abstract: Objective: To value the function of ligustrazine(川芎嗪) combined with albumin in curing acute cerebral hemorrhage after microtrauma clearing. Methods: Forty-five acute cerebral hemorrhages after microtrauma clearing patients were randomly divided into two groups. Medical treatment group was given ligustrazine and albumin. The comparative group was given conventional therapy. The damage grades of nerval function after the operation (1 and 15 days), the size of hydrocephalus and clinical effects were observed in the two groups. Results: The clinical effects, the damage grades of nerval function and the size of hydrocephalus were significantly different in the two groups (all P<0.01). In medical treatment group which was given ligustrazine and albumin, the total effective rate was 92 percent, and the size of hydrocephalus(15 days after operation) was (2.43±0.35)cm2, and the damage grades of nerval function was (11.44±3.75)scores. The results were superior to those respectively in comparative group 〔according to the order: 65 percent, (3.08±0.87)cm2, (18.55±4.74)scores〕. Conclusion: Ligustrazine combined with albumin can reduce hemorrhage after microtrauma clearing and pressure to the nearby organization effectively. It can also improve the curing rate and reduce the mortality.

    Effect of ligustrazini(川芎嗪) on xanthine oxidase activity during hepatic ischemia/reperfusion injury

    Abstract: Objective: To explore the effect of ligustrazini (川芎嗪,LGT) on xanthine oxidase (XO) activity during hepatic ischemia/reperfusion injury (HIRI). Methods: HIRI model in rabbits was used. Rabbits were randomly divided into three groups (n=10) and patients with liver cancer were randomly divided into two groups (n=9). The dynamic changes of XO activity in plasma and the effect of LGT on it were observed during HIRI in 30 rabbits and 18 patients who were scheduled for selective hepatic surgery. Results: XO activity significantly increased (P<0.05 and P<0.01) in the plasma and liver tissue during HIRI. After treatment of LGT, the abnormal change of the XO activity was alleviated remarkably and there were significant differences (P<0.05 and P<0.01) about XO activity in the plasma and liver tissue. Conclusion: These date suggest that LGT can effectively inhibit XO activity and decrease the formation of oxygen free radicals during HIRI.

    Clinical study on relationship between alteration of thyroid hormone and syndrome of traditional Chinese medicine in patients on hemodialysis

    Abstract: Objective: To analyse the relationship between alteration of thyroid hormone and syndrome of traditional Chinese medicine(TCM) in patients on maintenance hemodialysis (HD) to guide syndrome differentiation and typing, and to investigate its mechanisms. Methods: Total tri-iodothyronine (TT3), total thyroxine(TT4), free tri-iodothyronine(FT3), free thyroxine(FT4) and thyroid-stimulating hormone(TSH) of 60 cases who were on HD were detected. Influence of HD frequency on thyroid hormone was observed. Clinical symptoms were analysed to find relationship between alteration of thyroid hormone and syndrome of TCM. Results: TT3 and FT3 had significant differences in the group treated three times every week in comparison with the group less than three times(P<0.05). TT3, TT4, FT3, FT4 both in group of deficiency of Yang(Qi)〔偏阳(气)虚〕and group of deficiency of Yin(blood)〔偏阴(血)虚〕 had significant differences in comparison with normal group (all P<0.05). TT3, FT3 in group of deficiency and excess (虚实错杂) had significant differences in comparison with normal group (both P<0.05). TT3, TT4, FT3, FT4 in group of deficiency of Yang (Qi) had significant differences in comparison with both group of deficiency of Yin(blood) and group of deficiency and excess(all P<0.05). TT3, TT4, FT3, FT4 in the group of excess had not significant differences in comparison with normal group(all P>0.05). Conclusion: Thyroid hormone has relationship with deficiency and excess of body. Level of thyroid hormone can be regarded as index for guiding syndrome differentiation and typing, treatment or so. There is high meaning for improving malnutrition and reducing death rate. HD fully is favourable to improving secondary hypothyroidism.

    Abstract: Objective: To investigate the mechanism of traditional Chinese medicine Shenshuaikang liquid(肾衰康) improving chronic renal failure. Methods: A rat model of chronic renal failure was established by 5/6 renal resection, another group was given sham operation. The rats were randomly divided into three groups (the treatment group, the control group, the sham operation group). The treatment group was given Shenshuaikang liquid, the control group and the sham operation group was given water. The content of blood urea nitrogen (BUN), serum creatinine (SCr), transforming growth factor-β1(TGF-β1), fibronectin(FN), laminin (LN) and renal pathological change were observed in the three groups after 90 days. Results: Compared to those in the control group, the concentration of BUN and SCr were lower, they were (11.26±2.96)mmol/L vs. (18.33±3.93)mmol/L and (95.26±7.99)μmol/L vs. (127.90±23.29)μmol/L (all P<0.01); serum TGF-β1 and FN levels were decreased,they were (4.59±0.90) μg/L vs. (8.16±3.14) μg/L and (9.76±1.88)mg/L vs. (32.11±12.50)mg/L (all P<0.01); serum concentration of LN reduced, they were (16.67±9.65)μg/L vs. (22.27±6.80)μg/L (P<0.05). Renal interstitial in the control group increased and complicated with significant fibrosis. On the contrary the renal pathological change was slight in the treatment group; renal interstitial had no fibrosis but a few glomeruli segmental mesangial proliferation and segamental sclerosis. Conclusion: Shenshuaikang liquid can decrease the serum concentration of TGF-β1, FN and LN, and then ameliorate the renal damage. It can also inhibit interstitial fibrosis and prevent the progress of chronic renal failure.

    Effect of tea pigment(茶色素) to the endothelin and renal function in patients with primary nephritic syndrome

    Abstract: Objective: To study the effect and mechanism of tea pigment(茶色素) on endothelin (ET) in plasma and urinary and renal function of primary nephritic syndrome(PNS) patients. Methods: Sixty-five cases of PNS were randomly divided into therapeutic group (33 cases) and control group (32 cases). The control group was treated by normal treatment, and the therapeutic group was treated by tea pigment on the basis of normal treatment, and tea pigment was given 240 mg three times daily. The therapeutic course was 8 weeks both in two groups. The blood and urine ET and the change of the renal function were observed. Results: The blood and urine ET of PNS patients in both two groups were obviously higher than those of the healthy people before the treatment (both P<0.01). After the systematic treatment, the blood and urine ET of PNS patients had been obviously decreased in both two groups. But, the urine and blood ET of PNS patients in therapeutic group decreased significantly than those in control group. The differences between two groups were significantly (P<0.05 and P<0.01). After the treatment, the renal function in both two groups had been greatly improved. But, the blood urea nitrogen and creatinine decreased more obviously than those in control group (both P<0.05). Conclusion: On the base of general treatment, adding tea pigment to cure PNS will decrease the ET in blood and urine, improve and protect the renal function.

    Observation on curative effect of treatment for cerebral hemorrhage induced by hypertension in 60 cases with complex therapy

    Abstract: Objective: To observe the curative effect of the complex therapy in treating cerebral hemorrhage induced by hypertension and look for an effective therapy. Methods: One hundred and twenty patients with cerebral hemorrhage induced by hypertension were randomly divided into two groups. The 60 patients in complex therapy group was treated with complex therapy including microwounded operation of encephalic hematoma, rehabilitation treatment, western medicine and traditional Chinese medicine Xuezhong xiao(血肿消). The 60 patients in western medicine group were treated with microwounded operation and western medicine. Their periods of treatment were 28 days. The curative effects were compared between two groups. Results: Clinical heal rate and totally effective rate were 43.33 percent and 96.67 percent in complex therapy group, they were superior to those(30.00 percent, 88.33 percent) in western medicine group (both P<0.05). The absorption rate of hematoma was (98.17±6.32) percent in the complex therapy group, it was also superior to that (87.60±5.48) percent in western medicine group (P<0.05).Conclusion: Complex therapy could increase survival rate and improve quality of life, decrease the diseases' course and disability incidence.

    Effect of treatment with Zhengxintai(正心泰) on left ventricular function and late potential in patients with acute myocardial infarction

    Abstract:Objective:To study the electrophysiologic mechanism of Zhengxintai (正心泰) in treatment of acute myocardial infarction (AMI).Methods:The 59 patients with AMI were randomly divided into treatment group (treatment with western medicine and Zhengxintai) and control group (treatment with ordinary western medicine).The indexes of left ventricular function and late potential (VLP) were examined by the echocardiogram (ECG) and the ECG workstation. Results:Every indexes of treatment group after treatment were significantly better than those in control group (all P<0.05). The negative rate (26 percent) of VLP in treatment group was lower markedly than that in control group (18 percent, χ2=4.71, P<0.05), and the others indexes of VLP were superior to control group (P<0.05 or P<0.01). Conclusion:Zhengxintai may ameliorate left ventricular function and reduce the positive rate of late potential of AMI patients.

    Abstract: Objective: To compare the clinical effect of reinforcing kidney and activating blood serial recipe(补肾活血组方,RKASR) and gestrinone for the treatment of endometriosis (EMT) after operative laparoscopy, and explore the mechanism of RKASR in treatment of EMT. Methods: Sixty-two EMT patients were randomly divided into the treated group (n=32) which treated with RKASR, and the control group (n=30) which treated with gestrinone. Patents in both groups were treated with operative laparoscopy. The clinical symptoms, pregnancy rate, prostaglandin content in blood, hemorrheology and adverse reaction were observed in both groups. And the changes of CA125 before and after treatment in the treated group were compared. Results: The cure rate, pregnancy rate and the change of hemorrheology in the treated group were all better than those in the control group(P<0.01 or P<0.05), and the CA125 content in the patients was evidently decreased(P<0.01) after treatment with RKASR, but the prostaglandin content had not been decreased in treated group compared with that in control group(P>0.05). Conclusion: RKASR has definite effect to EMT patients, and its adverse reaction rate was lower than that of gestrinone.

    Effect of breviscapini injection (灯盏花素注射液) on the hemodynamic and heart function in chronic cor pulmonale patients

    Abstract: Objective: To investigate the effect of breviscapini injection(灯盏花素注射液) on hemodynamic and heart function in chronic cor pulmonale patient. Methods:Eighty-four cases were randomly divided into two groups: treatment group(46 cases) and control group (38 cases). All patients were treated with antibiotic, relieving cough and asthma, and resolving sputum medicine. Patients with heart failure were treated with digoxin or diuresis. Besides above treatment, the treatment group was treated with breviscapini injection 50 mg once a day intravenously. Two groups were treated for 20 days and clinical efficacy, heart function, hemorrheology, blood-gas analysis were evaluated. Results: ①The rate of controlled, notable efficacy, improved, fail and total effectiveness in the treatment group were respectively 17.39 percent, 45.65 percent, 30.43 percent, 6.52 percent, and 93.48 percent, while in the control group they were respectively 5.26 percent, 31.58 percent, 39.47 percent, 23.68 percent and 76.32 percent, the efficacy of the treatment group were superior to those in the control group(u=14.21988, P<0.05). ②The improvement of heart function in the treatment group was better than those in the control group (u=14.2109, P<0.05). ③In the treatment group, the blood viscosity, high shear rate viscosity, low shear rate viscosity were decreased, partical pressure of oxygen in artery and arterial oxygen saturation were increased after treatment than those in control group (all P<0.01). ④In treatment group, the levels of mean pulmonary arterial pressure(mPAP), pulmonary vascular resistance(PVR), cardiac output(CO) and cardiac index(CI) were obviously improved compared to those in control group (all P<0.01). Conclusion:Breviscapini injection could remarkablely change the hemorrheology, decrease pulmonary artery hypertension, improve heart function and increase clinical efficacy of chronic cor pulmonale.

    Influence of different treating principles and herbal formulas on energy metabolism of brain tissue in rats with focal cerebral ischemia/reperfusion injury

    Abstract: Objective: To investigate the influence of different treating principles and herbal formulas for extinguishing wind and opening the aperture(熄风开窍), dredging Fu organs and dispersing phlegm(通腑化痰), clearing away heat, extinguishing wind and dredging the meridians(清热熄风通络) and for activating blood circulation to remove stasis(活血化瘀) on energy metabolism of brain tissue in rats with focal cerebral ischemia/reperfusion(I/R) injury. Methods: Rat model of cerebral I/R injury was established by the middle cerebral artery occlusion. Various groups of the rats models were respectively fed with wind-extinguishing and aperture-opening decoction(熄风开窍合剂,WEAOD), Fu organ-dredging decoction (通腑合剂,FODD), antelope horn and scorpion capsule (羚蝎胶囊,AHSC), stroke healing pill (中风回春丸,SHP), and ATPase activity in brain was observed in cerebral I/R model rats. Results: Activities of Na+-K+-ATPase, Ca2+-ATPase, Mg2+-ATPase were higher in group of AHSC and group of SHP than those in control group. Compared with control group, activities of Ca2+-ATPase and Mg2+-ATPase were increased significantly in WEAOD group. Significant difference was not found in ATPase activity between control group and FODD group. Conclusion: The herbal formulas guided by the treating principles of clearing away heat, extinguishing wind and dredging the meridians, activating blood and dispering phlegm, and extinguishing wind and opening the aperture have the function of regulating the cerebral ATPase activity in rats with focal cerebral I/R injury.

    Abstract: Objective: To explore the better treatment in the chronic renal failure (CRF) patients due to diabetic nephropathy (DN). Methods: Twenty-nine CRF patients due to DN were divided into four groups according to their traditional Chinese medicine differentiations: deficiency of spleen-Yang-Qi and kidney-Yang-Qi (脾肾气阳虚), deficiency of spleen-Yin-Qi and kidney-Yin-Qi(脾肾气阴虚), deficiency of liver-Yin and kidney-Yin(肝肾阴虚), deficiency of both Yin and Yang(阴阳两虚). They were treated by modified decoction of six noble drugs(香砂六君子汤) adding aucklandia root and amomum fruit to warming the kidney and strengthen the spleen, modified decoction of six drugs including rehmannia and adding Danshen and astragalus root(参芪地黄汤) to supplementing Qi nourishing kidney(益气滋肾), modified decoction of six drugs including rehmannia and adding wolfberry and chrysanthemum(杞菊地黄汤) to nourshing Yin and calming the liver(养阴平肝), bolus for tonifying the kidney-Qi(金匮肾气丸) and Dihuang Yinzi(地黄饮子) to nourshing Yin and Yang(阴阳两补). The cure groups were compared with the contrast groups(23 cases) in clinic syndromes, fasting blood sugar, 24 hours urinary protein excretion, serum creatinine(SCr), blood urea nitrogen(BUN), total cholesterol(TC), triglyceride(TG) before and after treatment. Results: The total effective rate in the cure group was 89.5 percent and in the control group was 65.0 percent, while the difference was significant(P<0.05). In the cure group, the fasting blood sugar, 24 hours urinary protein, BUN, SCr were all decreased after treatment (P<0.05 or P<0.01), the improvement of TC and TG were observed but the difference was not significant (both P>0.05). The routine western medicine could decrease the fasting blood sugar (P<0.01), but the effect of improving the others indexes was not significant. Conclusion: Integrated traditional Chinese and western medicine treatment shows better effect than the single western medicine in CRF patients due to DN.

    Abstract: Objective: To investigate the clinical effect and mechanism of Jinnaduo(金纳多) in treating delayed encephalopathy which is caused by acute carbon monoxide poisoning. Methods: Eighty patients with delayed encephalopathy resulting from acute carbon monoxide poisoning were randomly divided into treatment group and control group. Forty-three patients in treatment group were treated in Jinnaduo 70 mg intravenous drip for 20 days; 37 patients in control group were treated in venoruton 1 000 mg and citicoline 500 mg intravenous drip for 20 days. The concentration of malondialdehyde (MDA), uric acid (UA) and platelet aggregation test (PAGT) were tested. Results: Totally effective rate in treatment group was 88.4 percent; and in control group was 67.6 percent. The treatment effects in two groups had significant difference (P<0.05). The concentrations of MDA and UA in treatment group after treatment were lower than those before treatment (both P<0.05); there was not significant difference in control group (both P>0.05). PAGT in treatment group after treatment was lower than that before treatment (P<0.01); there was not significant difference in control group (P>0.05). Conclusion: Jinnaduo in treating delayed encephalopathy resulting from acute carbon monoxide poisoning has good effect and seldom side effect, which can be used extensively in clinic.

    Abstract: Objective: To investigate the different blood routine characteristics between severe acute respiratory syndrome (SARS) and acute respiratory infection (ARI) patients, and provide reference to earlier differential diagnosis. Methods: Set up a database of blood routine examination information of SARS and ARI at the time they first coming to emergency department. Then using descriptive statistics and deductive statistics (U test and Chi-square test) to analyze the data. Results: Leukocyte count, lymphocyte count, neutrocyte count, platelet count and platelet bulk ratio of SARS were lower than those of ARI in earlier period (all P<0.05). But neutrocyte proportion and lymphocyte proportion in both groups had not valuable difference (both P>0.05). Conclusion: Blood routine examination is one of the important evidences for SARS' clinical preliminary diagnosis in emergency department.

    Abstract: Objective: To explore the effect of Yinao Tongluo capsule(益脑通络胶囊) on the content of serum tumor necrosis factor-α(TNF-α) and interleukin-2(IL-2) induced by endotoxin in rats. Methods: TNF-α and IL-2 contents were measured at 2 hours after endotoxin was injected through femoral vein. Results: Yinao Tongluo capsule groups and Buchang Naoxintong(步长脑心通) group all could inhibit the secretion of TNF-α compared with control group(P<0.05 and P<0.01). Meanwhile Yinao Tongluo capsule groups and Buchang Naoxintong group all could inhibit the secretion of IL-2 compared with control group(P<0.05 andP<0.01). Conclusion: Yinao Tongluo capsule can decrease the level of TNF-α, and increase the secretion of IL-2.
