经导管法栓塞动脉导管未闭的临床应用(摘要)

    目的 目的:采用Duct-Occlud(pfm)弹簧栓子和Amplatzer(AGA)栓子经导管法栓塞动脉导管未闭的临床应用。

    方法:从1998年5月至1999年6月,30例动脉导管未闭的病人通过导管栓塞法治愈。男性10例,女性20例;年龄8个月~30岁,平均年龄9.2岁;体重9.5~55 kg,平均28.0±19.9 kg;肺动脉平均压27.2 mmHg(10~61 mm Hg,1 mmHg=0.133 kPa);动脉导管最窄内径3.9±1.8 mm(2.0~10.0 mm)。镜像型右位心和动脉导管未闭结扎术后再通各1例。采用双螺旋形弹簧栓子(pfm)与具有自身膨胀性,可重复定位的AGA栓子栓塞各15例。

    结果:全部病人均获成功,术后主动脉造影显示24例完全封堵,6例有少量造影剂通过,术后24小时彩色多普勒显示27例完全封堵,2例1个月后无血流通过。平均X线暴露时间为13.2分(9~30分),无并发症出现。

    结论:两种封堵器材能够栓塞大多数动脉导管未闭的病人,并且安全有效。AGA产品更适合中到大型动脉导管未闭,由于使用简便,操作过程和X线暴露时间均有缩短。此两种器材可用于较小儿童。栓塞术后即刻效果良好,远期结果尚需长期随诊。

    Clinical Assessment of Transcatheter Closure of the Patent Ductus Arterious (Abstract)

    Objective: To observe the efficiency of transcatheter closure of the patent ductus arterious (PDA) using Duct-Occluder (pfm) coil and Amplatzer device (AGA).

    Methods: From May 1998 to June 1999, 30 patients (10 males, 20 females) with PDA were treated by transcatheter closure. The patients′ mean age was 9.2 years (ranged from 0.8 to 30 yrs) and mean weight was 28.0±19.9 kg (ranged from 9.5 to 76 kg). The mean pulmonary artery presure was 27.2 mmHg (3.6 kPa, ranged from 10 to 61 mmHg). The mean PDA diameter at its narrowest segment was 3.9±1.8 mm (ranged from 2.0 to 10.0 mm). One of the patients had mirror dextrocardia and another had a residual shunt after surgical ligation. PDA was occluded with double-cone coil (Duct-Occluder pfm) in 15 cases, and with the self-expandable, respositionable AGA duct occluder device in the other 15 cases.

    Results: All patients were successful. Angiography showed that 24 cases had instantly complete closure, 6 cases had a small residual shunt. The color doppler revealed a complete closure in 28 cases 24 hours after the procedure and also after one month in 2 cases. The mean fluoroscopy time was 13.2 min (ranged from 9 to 30). There were no complications.

    Conclusion: Both of the occlusion device systems are safe and effective in closing of most patients with PDA. The AGA device is suitable especially to the moderate and large PDA. The operative and fluoroscopical time is shorter because it is easier to use. Both systems are allowed to use in small children. The instant effect is good, while the remote effect still needs a long term follow-up., 百拇医药
