【摘要】 目的 探讨血管蒂髂骨瓣移植治疗股骨头缺血性坏死的治疗方法。方法 在股骨头颈交界处前上方开窗，用自制磨钻在直视镜下将股骨头的坏死骨彻底磨除，将带血管蒂的髂骨块修整好移植到掏空的股骨头内共121例，150髋。结果 诊观察6个月～8年，按赵德伟的100分评分标准，FicatⅠ期优99%，Ⅱ期优良率92.5%，Ⅲ期优良率74.6%，Ⅳ期优良率51.3%，总的优良率87.5%。结论 带血管蒂骨瓣移植术为目前治疗中、青年股骨头缺血性坏死主要方法，其中以带旋股外侧动脉升支复合组织蒂髂骨瓣为首选。
