【摘要】 目的 分析腰椎间盘突出症临床定位与影像学检查分离的原因及探讨其治疗方法。方法 回顾总结手术治疗临床定位间隙与影像学检查同一间隙出现截然相反情况的10例患者，分析引起这种情况的原因。结果 平均随访2年2个月，优良率达90%。结论 对于腰椎间盘突出症临床定位与影响像学检查分离的患者有反复发病史，症状、体征重，影响患者的工作及日常生活，保守治疗无效时，应采取手术治疗，行双侧椎板开窗或半椎板切除，在合并侧椎管狭窄时应扩大减压，同时注意上或下腰椎间盘突出的影响。
