【摘要】 目的 为了评价4.2%高渗氯化钠羟乙基淀粉40治疗出血性休克病人的临床疗效。方法 选择均为创伤出血性休克病人。采取多中心临床试验、开放、前后自身对照方法。高渗氯化钠羟乙基淀粉40溶液8ml/min/kg静脉快速输注，给药后5～90min监测各时期血压、尿量、心率、肝肾功能。评价临床疗效标准:分别取邻近两次收缩压达到100～120mmHg时停止用药。结果 观察20例出血性休克病人，11例(55%)收缩压在给药后5min开始升高。7例(35%)在给药后10min血压开始升高。平均给药时间为(34.5±15.72)min，17例(85%)收缩压在用药后15～50min达到正常标准，P<0.01。输入高渗氯化钠羟乙基淀粉40平均剂量为(402.00±148.20)ml，施用药物后30min病人平均尿量为4.74ml/h/kg。结论 4.2%高渗氯化钠羟乙基淀粉40能够扩充血容量、提高血压、改善出血性休克病人的血流动力学。
