【摘要】 目的 探讨原发性高血压患者血管紧张素Ⅱ受体-1(AT1R)基因多态性与双氢克尿噻降压疗效的相关性。方法 200例未服用过任何药物或者已停用抗高血压药物两周的原发性高血压患者入选本实验，其中符合研究目的且资料完整的共计177例，入选人群同时开始服用双氢克尿噻12.5mg/日，共6周，在服药前及服药后每两周记录血压数值一次。用聚合酶链反应(polymerase chain reaction，PCR)及限制性酶切方法进行AT1R基因A1166-C位点的多态性分布检测，并测定入选者的空腹血糖、总胆固醇、甘油三酯和高密度脂蛋白。入选人群按AT1R基因多态性分为AA、AC基因型两组。结果 双氢克尿噻对于AA及AC基因型原发性高血压患者的收缩压和舒张压均有明显的降压效果(P均<0.05)，并且这种降压效果主要表现在用药后的前两周内。AA及AC基因型患者的收缩压下降程度的比较无统计学意义(P>0.05)，而AC基因型患者的舒张压下降程度与AA基因型患者的比较差异有显著性(P<0.05)。结论 双氢克尿噻对于原发性高血压患者舒张压的降压效果可能与AT1R的基因型有关。

    【Abstract】 Objective To study the correlation of AngiotensinⅡTypeⅠReceptor(AT1R)Gene A1166C Polymorphism with the effect of antihypertensive treatment by Hydrochlorothiazide in essential hypertensive patients.Methods 177hypertensive subjects who were untreated or who had untaken medicine for two weeks were enrolled.Blood pressure was measured before taking medicine and was examined every two weeks after taking Hydrochlorothˉiazide12.5mg per day for six week.Polymerase Chain Reaction(PCR)combined with restriction enzyme digestion was used to detect the variation of A1166C allele.Fasting blood glucose and serum cholesterol、triglyceride and high density lipid were measured in the enrolled subjects.The patients were characterized according to the observed alleles as AA or AC.Results The reduction in SBP was no statistical significant difference between AA and AC groups(P>0.05).But the reduction in DBP of AC group was higher than that of AA droup(P<0.05).Conclusion There may be correlation of AT1R A1166C polymorphism with the effect of antihypertensive treatment in DBP by Hyˉdrochlorothiazide in essential hypertensive patients. ......
