【摘要】 目的 通过对459例妊娠妇女临床观察，了解糖化血红蛋白(glycosolated hemoglobin A 1c ，HbA 1c )在妊娠期糖尿病(GDM)筛查中的作用，以及对具有高危因素妊娠妇女早期糖筛查的意义。 方法 用HbA 1c 、空腹血糖和糖负荷实验(GCT)三种方法对进行筛查，详细记录妊娠妇女年龄、孕前体重、体重指数(BMI)和糖尿病家族史。 结果 早孕期进行GDM的筛查，可尽早发现GDM;比较GDM组和正常组在年龄、孕前体重、体重指数(BMI)和糖尿病家族史差异有显著意义(P<0.05);HbA 1c 为5.15%时，灵敏度为95.5%，特异度为84.1%，灵敏度较高，而空腹血糖为4.8mmol/l时灵敏度为72.7%，特异度为89%，灵敏度差。 结论 在围产期门诊开展对早孕病人的GDM筛查，加强对具有高危因素病人的筛查，对及早发现病人，及时控制病情，减少GDM并发症很有帮助;HbA 1c 的测量很可能也是一种较为实用GDM的筛查方法。
