【摘要】 目的 利用高分辨率超声探讨侧胸皮瓣成型术前及术后的血流动力学的变化特点，判断皮瓣的预后。 方法 采用Acuson Sequoia512血流显像仪行二维彩色血流分布特征检测胸外侧动脉及其皮支，再采用彩色多普勒(PWD)探测拟行皮瓣区血管的血流动力学指标。手术后2h、1天、3天、5天、7天、12天、21天、30天运用彩色多普勒超声仪探测皮瓣区二维彩色血流分布状况及血流动力学变化，术后30天，切取整块皮瓣行皮瓣墨汁灌注。 结果 Vmax、Vmean术后2h略高于术前水平，术后1～7天明显高于术前水平，并于5～7天达峰值流速，术后12、21天呈明显下降趋势，术后第30天基本恢复术前水平;PI、RI、S/D术后1～7天虽低于术前水平，但呈逐渐上升趋势，术后30天基本恢复术前水平。侧胸皮瓣成形术后5天，皮瓣成活面积100%。 结论 应用高分辨率超声，对于术后动态监测皮瓣区微循环血供变化，判断皮瓣的预后及早发现血管危象具有重要临床意义。
