【摘要】 目的 通过螺旋CT扫描三维影像(3D)和二维影像(2D)在寰枢椎骨折和(或)脱位中的应用，评价螺旋CT扫描的优越性，比较3D和2D影像诊断的敏感性及准确性并探讨3D和2D影像的临床价值。 方法 对临床诊断有寰枢椎骨折和(或)寰枢关节脱位的82例外伤患者进行薄层螺旋CT扫描三维重建和多平面重建。 结果 3D图像光滑、细腻、逼真，清楚显示骨折线走行方向、骨折范围、位置、类型、骨折块移位情况及与周围骨骼的解剖关系，对骨折显示立体直观，能清楚显示骨折、脱位的畸形表现等空间关系。2D影像对细小骨折显示较好，且可同时显示骨折部位的软组织改变情况及脊髓受压情况。 结论 螺旋CT扫描方式可获得高质量3D图像，3D影像在空间信息方面明显优于2D影像，其临床价值明显大于2D影像，3D图像显示对临床处理有较大的指导作用。而2D影像是3D图像重建的基础，两者在临床应用中各有优缺点，两者互相结合可以提高对寰枢椎骨折及脱位显示的准确率。
