【摘要】 目的 比较分析PPH术与传统Milligan术治疗重度痔(Ⅲ、Ⅳ期)的临床效果。 方法 142例Ⅲ、Ⅳ期痔患者，男82例，女60例，PPH术治疗组72例(PPH组)，Milligan术治疗组70例(Milligan组)。 结果 手术时间PPH组(16.08±4.52)min，Milligan组(30.24±7.54)min，P<0.01。术中出血PPH组(20.42±8.43)ml，Milligan组(38.42±9.47)ml，P<0.05。术后使用止痛药，PPH组8例(11.11%)，Milligan组14例(20%)，P<0.01。尿潴留PPH组18例(25%)，Milligan组14例(20%)，P>0.05。住院时间PPH组(4.22±1.24)d，Milligan组(7.42±3.43)d，P<0.01。患者满意度，PPH组98%，Milligan组96%，P>0.05。复发率PPH组4例(5.56%)，Milligan组6例(8.57%)，P>0.05。肛门狭窄PPH组2例(2.78%)，Milligan组8例(11.43%)，P<0.01。 结论 PPH治疗Ⅲ、Ⅳ期痔具有手术时间短、术后疼痛轻、恢复快、并发症少等优点，将成为重度痔的首选治疗方法。
