摘要 目的：探讨乳腺癌在MPMNs中的地位。方法：回 顾性分析1958年～1997年收治的398例乳腺癌伴MPMNs。结果 ：1)双原发乳腺癌发生率为2.5%，发病年龄41岁，间隔 时间5.4年。5、10、20年生存率分别为86.6%、57.7%和25.8% 。2)同一乳腺PD伴乳腺癌发病率为0.9%，高发年龄为 47.3岁。5、10、20年生存率为：64.7%、50%和20.6%。3)乳 腺癌伴其他脏器癌瘤的MPMNs发生率为3.6%，高发年龄 49.4岁，间隔时间为8.6年。好发器官为肺、子宫、食管 、卵巢和大肠。4)乳腺癌作为第二、三原发癌的MPMNs发 生率2.3%，年龄50.1岁，间隔时间7.1年；第一原发癌好 发器官为：子宫、大肠、卵巢和食管。结论：1)乳腺 是MPMNs的主要受累器官。2)乳腺癌伴发MPMNs中易受累的 器官为：女性激素的靶器官(乳腺、子宫、卵巢) 和乳腺癌放疗区域的器官(肺和食管)。3)正确诊断 和治疗可得到较好的治疗效果。

Breast Cancer Associated with Multiple Primary Malignant Neoplasms

Abstract Objective: To study the role of breast cancer in multiple primary malignant neoplasms (MPMNs). Methods: Three hundred and ninety- eight cases of breast cancer associated with MPMNs were analysed retrospectively. Results: 1) The incidence rate of primary bilateral breast cancers was 2.5% with the morbidity age of 41 years, intermitting time between the two malignancies was 5.4 years. The 5- ,10- and 20- year survival rates were 86.6% , 57.7% and 25.8% respectively. 2) The incidence rate of Paget′ s disease associated with breast cancer was 0.9% with a high incident age of 47.3 years and 5- , 10- and 20- year survival rates of 64.7% , 50% and 20.6% respectively. 3) The incidence rate of breast cancer associated with MPMNs of other organs was 3.6% with the peak incident age of 49.4 years, intermitting time of 8.6 years and the organs commonly involved were lung, uterus, esophagus, ovary and large intestine. 4) The incidence rate of breast cancer as the second or third primary malignancy associated with MPMNs was 2.3% with the high morbidity age of 50.1 years, intermitting time 7.1 years and the commonly involved organs were uterus, large intestine, ovary and esophagus. Conclusion: 1) Breast was the main organ of MPMNs. 2) The predilection organs of MPMNs of breast cancer were the target organs of female hormone (breast, ovary and uterus) and the organs in the radiative area of breast cancer (lung and esophagus). 3) Correct diagnosis and treatment strategy of the disease can get better results.
