摘要 目的: 了解白血病患儿巨细胞病毒(CMV)肺炎的发生状况及CMV抗原血症检测方法对CMV肺炎的诊断意义。方法: 实验组为31例白血病患儿,对照组为31例免疫功能正常的儿童,应用间接免疫荧光方法检测外周血白细胞CMV抗原;应用ELISA方法检测血清抗CMV-IgM、IgG抗体。结果: 白血病患儿中有9例发生活动性CMV感染,感染率为29.1%,对照组为0,两者之间差异显著(P<0.01)。9例活动性CMV感染患儿中,有6例为肺炎,临床表现无特异性,胸片表现多样化,病情轻重不一。6例CMV肺炎患儿检测CMV抗原血症均为阳性。结论: 白血病患儿易发生CMV肺炎。CMV抗原血症检测技术可早期诊断白血病患儿CMV肺炎,为临床早期治疗提供实验依据。

Abstract Objective : To evaluate the incidence and clinical course of cytomegalovirus(CMV) pneumonia in leukemia children and the significance of CMV antigenemia assay for diagnosing CMV pneumonia.Methods : Thirty one leukemia children and 31 immunocompetent control children were involved in the study. Indirect immunofluorescence assay was used for detecting CMV antigen in peripheral blood leukocytes,ELISA for anti-CMV IgM and IgG.Results : Active CMV infection was diagnosed in 29.1%(9/31) leukemia children while none in control group.Active CMV infection rate showed a significant difference between these two groups(P<0.01).Six out of nine CMV infected patients had CMV pneumonia during the period of induction phase.There were no specific clinical patterns of CMV pneumonia,but the chest X-ray showed different manifestations and different severity.CMV antigenemia occurred in all of the six patients with CMV pneumonia.Conclusion : CMV infection was common in leukemia children.Pneumonia is one of the most common diseases of CMV infection.CMV antigenemia assay is a rapid diagnostic technique which can identify patients likely to develop CMV pneumonia,potentially allowing early treatment.
