摘要】 目的 研究血管内超声对冠状动脉内斑块形态的分析,以及斑块形态与患者临床表现的关系。方法 对42例冠心病患者行冠状动脉造影及冠状动脉内超声成像检查(ICUS),定性分析斑块形态和性质,并与冠状动脉造影对比。患者根据临床特征分为不稳定型心绞痛组(A组)和稳定型心绞痛组(B组)。结果 冠状动脉造影发现,冠状动脉狭窄程度两组比较无显著性差异,而对比两组超声斑块形态和性质发现A组病变以软斑块多见(A组66%,B组31%,P<0.05),B组则以纤维斑块更常见(B组54%,A组17%,P<0.05)。结论 ICUS对病变形态和性质的分类与临床综合征关系更密切。Intracoronary ultrasound imaging correlation of plaquemorphology with clinical syndrome and angiographyZeng Yong, Zhu Wenling, Huang Chaolian,et al. 【Abstract】 Objective To establish therelationship between ultrasound derived atheroma morphology and angiography with clinicalsyndrome. Methods Forty two patients were examinedby intracoronary ultrasound imaging (ICUS) and coronary angiography. Ultrasound imageswere analyzed for percent plaque areas and classified into four morphologic subtypes(soft, fibrous, calcific, mixed plaque). Ultrasound data were compared with angiographicfeature and clinical angina pattern.Results Ultrasoundlesion morphology well correlated with the clinical angina syndrome. Compared with stableangina, patients with unstable angina had more soft lesions (66% vs 31%, P<0.05), lessfibrous lesions (17% vs 54%, P<0.05). But the degree of stenosis shown angiographicallyrevealed no significant difference between patients of stable and unstable angina.Conclusion ICUS can detect early process ofatherosclerosis. Ultrasound classification of plaque morphology is closely correlated withclinical angina sysdrome.
