【摘要】 目的 观察人体静注利多卡因对脑氧耗和脑血管功能的影响。方法 20例颅脑手术病人,麻醉前经颈内静脉和桡动脉穿刺置入导管。在静注2%利多卡因1.5mg/kg前后2分钟分别从导管中取动、静脉血标本检测血气。并用HNCPA-2脑血管功能检测仪记录双侧颈内动脉和椎动脉的分钟血流量、血流入时间和血管转折高比率。用公式计算脑氧耗量。结果 注药后脑氧耗量降低17.6%,入颅动脉血管弹性降低9.8%,血液流入阻力增加17.6%,脑血流量减少13%。结论 单次静注利多卡因后出现脑血管收缩,入颅血流量和脑氧耗量降低,有利于降低颅内压和保护颅脑手术中缺血脑细胞的功能。
Effect of lidocaine on cerebral oxygen consumption and cerebrovascular function

【 Abstract 】 Objective To assess the protective effects of lidocaine on cerebral oxygen consumption and cerebrovascular function.Methods Internal jugular vein (IJV) and radial artery (RA) were cannulated in 20 patients before anesthesia induction. PO2 an SO2 of IJV and RA blood ,bilateral cerebrovascular function were determined 2 min before and after the intravenous administration of 2% lidocaine 15 mg/kg respectively,the cerebral oxygen consumption (COC) was then caculated.Results Two min before and after lidocaine administration, cerebral blood flows were 547 and 473 ml/min , the values of COC were 33.98 ml and 27.94 ml ,blood flow times were 0.17s and 0.20s, and the vascular elasticity were 0.81 and 0.73, between which there were significant differences (P<0.05 or 0.01).Conclusion Lidocaine can decrease cerebral metabolism,increase cerebral blood flow resistance and reduce cerebral vascular elasticity,which may be beneficial for reduction of intracranial pressure and recovery of ischemic brain function in craniotomy.
