【摘要 】 目的 探讨大肠癌患者外周血微转移的检测及临床意义。方法 以CK20非同位素RT-PCR方法，检测54例大肠癌患者术前和术后不同时间外周血中结肠癌细胞。结果 54例病人外周血中查出CK20阳性表达32例，阳性率59.63%，CK20阳性检出与Duke’s分期、淋巴转移存在差别有显著性。术后3天内外周血CK20阳性检出较术前多出3例，术后早期短程化疗结束后16例转阴。结论 大肠癌在早期就可能已累及全身，外周血中CK20的检测可帮助综合判断疾病恶性程度及患者预后，为指导辅助化疗提供了理论依据。术后早期短程化疗对控制大肠癌微转移具有一定的作用。

Detection of cancer cells inperipheral blood of patients with colorectal carcinomas and its clinical implication

【Abstract 】 Objective To study the detection of micrometastasis in peripheral blood of patients with colorectalcarcinomas and its clinical implication. Method Cytokeratin 20 (CK20)mRNA in peripheral boold of 54 patients with colorectal carcinomas was detected at varioustime points before and after operation by using non-isotope RT-PCR technique. Results The positive rate of CK20 mRNA in peripheral blood was 59.63%. The positive detection wassignificantly correlated with Dukes stage and liver metastasis. There were more 3 positivecases in 3 days of postoperation. After a short-term chemotherapy in the early stage of postoperation, 16 cases transformed from CK20 positive to negative. Conclusions The detection of CK20 in peripheral blood was helpful to the comprehensive judgment ofmalignant extent and prognosis. The short-term chemotherapy in the early stage ofpostoperation plays a role in controlling the micrometastasis of colorectal carcinoma.
