【摘要】 目的 探讨血清和尿中白细胞介素-8(IL-8)水平变化在系统性红斑狼疮(SLE)及狼疮肾炎(LN)中的临床意义。方法 采用ELISA法对47例SLE患者(含37例LN)、13例原发性慢性肾小球肾炎和14例正常对照进行血清和尿中IL-8水平检测，比较分析其与SLE活动指数(SLEDAI)、肾脏损害的相关关系。结果 SLE患者血清IL-8水平活动期为55.3pg/ml(中位数，下同)静止期为26.38 pg/ml，明显高于正常对照组20.54pg/ml(P<0.001)，并且活动期高于静止期(P<0.05)，血清IL-8水平与SLEDAI正相关(r=0.6003,P<0.05);SLE患者尿IL-8显著高于正常对照组20.67pg/ml(P<0.05)，且LN组25.29 pg/ml明显高于SLE无肾炎组为21.1 pg/ml及原发性慢性肾小球肾炎组19.41pg/ml(P值均<0.05)；肾脏组织病理狼疮活动指数(AI)高者其血清、尿IL-8水平显著高于AI低者(P<0.05)。结论 血清IL-8水平可作为SLE活动度的敏感指标之一，而尿IL-8升高有助于判断LN患者肾脏的炎症活动情况。

【Abstract 】 Objective Toinvestigate the clinical significance of serum and urine interleukin-8(IL-8)levels inpatients with systematic lupus erythromatosus(SLE) as well as lupus nephritis(LN).Methods ELISA assay was employed toevaluate serum and urine IL-8 levels of 47 SLE patients including 37 LN,13 primary chronicnephritis patients and 14 normal control people. Results Serum IL-8 level of SLE patients was higher than that of normalcontrol group(P<0.001). In addition, Serum IL-8 level of active SLE was higher thanthat of quiescent SLE (P<0.05). The level of serum IL-8 in SLE patients significantlycorrelated with SLEDAI scoring(r=0.6003,P<0.05).Urinary IL-8 level of SLE patients washigher than that of normal control group(P<0.05). Moreover,urinary IL-8 level of LN wassignificantly higher than that of non-renal involvement SLE and primary chronicnephritis(P<0.05).Both serum and urine IL-8 levels in patients with renal activityindex(AI) ≥8 were significantly higher than those inpatients with AI<8(P<0.05).Conclusion Serum IL-8 level could sensitively reflect SLE disease activity andurinary IL-8 level help to evaluate the inflammation of the kidney.
