【摘要】 目的 探讨门脉高压时胰岛功能的变化。方法 应用放免法检测肝硬化门脉6高压、肝前性门脉高压及正常大鼠循环血液中胰岛素及胰高血糖素水平。采用非酶法分离并纯化以上三组大鼠胰岛,检测其体外生物学活性。结果 二种门脉高压模型鼠循环血液中胰岛素及胰高血糖素水平均明显高于正常对照组(P<0.01);其分离纯化的胰岛经体外培养48小时及在含糖基质中孵育2小时后,胰岛素释放量明显低于正常对照组,胰高血糖素释放量明显高于正常对照组(P<0.01)。结论 门脉高压时:① 循环血液中胰岛素及胰高血糖素水平增加;② 胰岛α细胞分泌增加可能有助于门脉高压高胰高血糖素血症的发生;③ 循环血液中胰岛素水平增加,但胰岛β细胞功能明显低于正常。
Study on the impaired function of pancreatic islets in portal hypertensive rats

【 Abstract】 Objective To find out the change of the pancreatic islets function in portal hypertension.Method The plasma concentration of insulin and glucagon were measured by RIA in cirrhotic, partial portal vein ligated and normal rats. Pancreatic islets from normal, partial portal vein ligated and cirrhotic rats were isolated by nonenzymatic mechanical methods. Insulin and glucagon secretion were measured in vitro in isolated islets.Results Both rats with partial portal vein ligation and those with cirrhosis caused by carbon tetrachloride exhibited hyperinsulinemia and hyperglucagonemia as compared with control rats(P< 0.01). Isolated islets from the two experimental portal hypertensive models showed impaired insulin secretion after culture for 48 hours or incubation in the presence of glucose for 2 hours, on the contrary, glucagon secretion incrased significantly.Conclusion our data show that hyperglucagonemia in portal hypertensive rats is promoted by an enhanced alpha-cell secretion. The level of serum insulin is high, however, secretory function of beta-cell is low.
