【摘要】 目的 为研究中国人男性激素模式与体脂分布的关系。方法 用核磁共振(MRI)测量88名中国男性的体脂分布,并分为腹内脂肪积聚与非腹内脂肪积聚组。同时测定睾酮(TT)、游离睾酮(FT)、硫酸去氢表雄酮(DHEA-S)、性激素结合球蛋白(SHBG)、生长激素(GH)、皮质醇(F)、空腹及糖负荷后胰岛素及C肽。结果 腹内脂肪增多者TT显著下降,不受BMI影响。皮质醇及糖负荷后2小时胰岛素、C肽显著增加。多元逐步回归分析见到,TT对腹内脂肪的影响达19.22%。结论 雄性激素减少是腹内脂肪积聚者的特殊改变。

【Abstract】 Objective To investigate the possible relationship between hormone pattern and body fat distribution in Chinese male individuals. Methods Regional adipose tissue distribution indices 〔including visceral adipose tissue areas (VA), abdominal subcutaneous adipose tissue areas (SA) and femoral subcutaneous adipose tissue areas (FA)〕 were measured by Magnetic Resonance Imaging (MRI) in 88 Chinese male individuals, meanwhile serum total testosterone (TT), free testosterone (FT), dehydroepiandrosterone sulfate (DHEA-S), sex hormone binding globulin (SHBG), growth hormone (GH), cortisol (F),fasting and post glucose loading plasma immunoreactive insulin and C-peptide were also measured in VA accumulation and non-VA accumulation groups. Results Adjusted with BMI, AGE, SA and FA, subjects with high VA had lower TT levels (P<0.05), higher F(P<0.01) , increased insulin (P<0.05) and C-peptide (P<0.01) levels 2h following an oral glucose challenge of 75 gram. Multiple stepwise regression analysis showed that the relativecontributionofTTandfastinginsulinlevelindetermining VA reached 19.22% (P=0.0001) and 11.30% (P=0.0004) respectively. Conclusion A lower androgen level is the characteristic change in Chinese male individuals with visceral adipose tissue accumulation.
