【摘要】 目的 探讨HCV核心基因变异在肝癌发生中的作用。方法 从HCV RNA阳性的肝病患者血清中提取HCV RNA,经随机引物逆转录合成cDNA,再对核心基因进行巢式PCR扩增,产物连接p-GEM-T载体,在大肠杆菌中表达后的38个克隆进行测序分析。结果 肝癌与肝炎患者的核心基因存在着不均一性,肝癌患者株的核苷酸置换较明显地高于慢性肝炎株。核苷酸的置换无规则地出现在整个核心基因区,但在核心基因编码的第二个亲水区域,错义突变较为集中。结论 提示HCV核心蛋白基因的变异,与HCV慢性持续感染及肝癌发生的关系密切。

【Abstract】 Objective To explore the etiological role of the core genemutations of hepatitis C virus (HCV) in the causation of hepatocellular carcinoma (HCC). Methods HCV RNAs extracted from the sera of patients with HCV RNA positive were converted to cDNAby reverse transcription with random primer, the core genes of HCV genome were amplified,the PCR products were expressed in E. coli with p-GEM-T plasmid vector, and 38 nucleotidesequences were determined by dideoxynucleotide chain-termination method. Results The genomic heterogeneity of HCV and the sequence divergence of the HCV core gene wereshown in individuals with chronic hepatitis (CH) and HCC. The rate of nucleotidesubstitutions in the core gene of HCV isolated from HCC patients was significantly higherthan that from CH. The nucleotide substitutions were unevenly scattered along the coregene; a cluster of missense mutations was apparent in the region encoding the secondhydrophilic domain of the core protein. Conclusion The present datasuggest that missense mutations in the core gene region are closely related with HCVchronic persistent infection and progression of chronic liver disease to HCC.
