【摘要】目的 本实验研究胫骨上端前后缘皮质骨对单髁假体稳定性的影响。方法 实验分为三组:A组,正常组;B组,假体前缘未压在皮质骨上;C组,胫骨上端后缘皮质骨破坏。结果 ⒈仅经一次循环,700~900N的负荷就使B组假体的前微动超过2mm,而A组假体经30次循环,981N的负荷使假体的前微动约为400μm。2.A组假体的前微动约为C组的二分之一,后微动约为C组的三分之一。结论 胫骨上端前后缘皮质骨对假体的稳定性非常重要。

    

Effects of Anterior andPosterior Cortical Bone of Proximal Tibia on the Initial Fixation Stability ofUnicompartmental Prosthesis

Abstract Objective Toinvestigate the effects of the anterior and posterior cortical bone of the proximal tibiaon the stability of the unicompartmental prosthesis. Methods The embalmedhuman anatomic- specimensof tibia were size- selectedand randomly divided into 3 groups.In group A,the fixation of the prosthesis was the sameas that used clinically. The anterior part of the prosthesis was placed on the anteriorcortical bone of the tibia with the posterior cortical bone intact.In group B, thefixation was similar to that of group A except that the anterior part of the prosthesiswas fixed on the cancellous bone.In group C,the anterior part of the prosthesis was fixedon the tibial surface including the anterior cortical bone, but the posterior corticalbone was destroyed. All of the components were implanted into the medial compartments ofthe tibiae. Results Group B was found to be the worst in the threegroups. At the first cycle, the micromotion of group B was higher than 2mm in response to700~ 900 N loads.Group A was found to be significantly better than group C,in response to 981 N load, themicromotion was approximately 400μ m. Its anterior subsidence was about one half of those ingroup C and its posterior lift off was about one third of those in group C.Conclusions Both of the anterior and posterior cortical bone of the tibia was very important to thestability of the unicompartmental component. The implants should be placed on the anteriorcortical bone of the tibia and the posterior cortical bone unbroken.
