【摘 要】目的： 为提高多指皮肤全脱套伤的治疗水平，设计双侧脐旁带血管蒂真皮下血管网皮瓣修复15例55指的多指皮肤全脱套伤。方法 ：以脐旁血管为蒂，在脐旁两侧各设计一皮瓣从伤指侧方呈横向包埋伤指，利用皮瓣的长度，使两伤指间预留足够宽的皮瓣以便分指时无需指侧植皮；供皮瓣区在9 cm内可直接拉拢缝合；术后11～14天断蒂，断蒂后14天行分指术，为早期伤指功能锻练创造了条件。结果 ：3指于断蒂处皮瓣去薄时小面积起水泡坏死，经换药治愈，52指皮瓣全部成活良好，1年后复查，所有伤指外形、功能良好。结论 ：该项技术具有皮瓣设计新颖，不需植皮，早断蒂，早分指，伤指修复外形、功能良好，住院时间短、节约经费等优点。

REPAIRATION OF DEGLOVING INJURY OF MULTIPLE FINGERS WITH BILATERAL PARAUMBILICAL ARTERY PEDICLE FLAP WITH SUBDERMAL VASCULAR NETWORK

【ABSTRACT】 Objective : To counter the disadvantages of repairing of multiple fingers with degloiving injury, a new method of repairation was reported. 15 cases with total of 55 gloving injury fingers were repaired by the bilateral paraumbilical artery pedicle flap with subdermal vascular network. Method : It was designed separately on the left and the right of the paraumbilical perofrators. Over 2 fingers were covered cross wisely with wide space of skin flap by cach lateral flap. Thus, widely space of skin flap was prepared between two injured fingers for the purpose of dividing finger early and not skin grafting on the lateral fingers. Division of pedicle was performed 10～14 days post-operation, then divided fingers 14 days after division of pedical. That made it possible physical training early. Results : All of donor areaes were direct sutured. All of skin flaps were survived. Conclusion : The advantages of this flap areas follows:(1) new design; (2) simple and reliable; (3) free rotation and comfortable fixation; (4) division of pedicle and dividing-finger early; (5) atisfying shape and function (6) reducing the operative number of times and saving money.
