【摘要】 目的 探讨三维CT重建对小儿寰枢椎旋转畸形的临床应用价值。方法 对7例患儿进行CT扫描和三维CT重建的检查。结果 三维CT重建清楚地显示枕寰枢椎骨结构的解剖和立体关系。当头旋向左侧时,寰椎右侧侧块移向前方和中线,而左侧侧块移向后内侧;前位显示寰枢椎同时向旋转侧倾斜;俯视位可显示齿状突倾斜角度及其与寰椎前弓之间的关系;仰视位可发现同侧寰枢椎横突孔距离不对称,旋转侧较健侧距离长。结论 三维CT重建能多方位、立体地显示枕寰枢椎关节的形态,测定旋转的角度,对了解小儿寰枢椎旋转畸形的寰枢椎骨结构解剖和病变有较大的临床价值。

【Abstract】 Objective To evaluate the role of three-dimensional CT(3-D CT) reconstructionin imaging atlanto-axial rotatory deformities in children. Methods Seven children with atlanto-axialrotatory deformities underwent CT scan and three-dimensional reconstruction. Results 3-D CT reconstruction clearlydemonstrated the anatomy of the bony structures and the spatial relationships between thevertebral bodies. With the head turned to the left side, the right transverse processmoved forwards and medially whereas the left transverse process rotated posterio-medially.Anterior view showed simultaneous tilt of both C1 and C2 towards theside of rotation. The cranial view showed the angle of inclination of the dens in relationto the anterior tubercle of atlas. The caudal view showed asymmetry in distance betweentransverse foramina of C1 and C2 . Conclusions 3-D CT illustrates the anatomicalrelationship of atlanto-axial complex and provides important information of the pathology.
