【摘要 】 目的 探讨慢性阻塞性肺疾病(COPD)和支气管哮喘患者气道分泌物中,感觉神经肽降钙素基因相关肽(CGRP)释放在气道慢性炎症性疾病发病中的作用。方法 选择稳定期COPD患者19例、支气管哮喘患者14例、正常对照组10名,通过面罩吸入超声雾化的4%高张盐水诱导痰液,将获得的诱导痰涂片进行瑞氏-姬姆萨染色计数细胞成分的变化,用放射免疫方法测定诱导痰中CGRP免疫反应性物质(CGRP-LI)的含量改变,并进行CGRP-LI与一秒钟用力呼气容积(FEV1 )的相关性分析。结果 诱导痰中CGRP-LI含量在COPD组为(15.97±2.15)ng/L,支气管哮喘组为(18.79±3.91)ng/L,与正常对照组(2.36±0.35)ng/L比较,差异有显著性(P<0.05),而且与FEV1 呈显著负相关(r=-0.50和-0.61,P均<0.05)。COPD患者诱导痰中中性粒细胞百分比为(64.9±2.9)%,与支气管哮喘患者(26.5±1.5)%和正常对照组(25.9±2.6)%比较,差异有显著性(P<0.01);支气管哮喘患者诱导痰中嗜酸细胞百分比为(5.80±0.50)%,与COPD患者(1.00±0.20)%和正常对照组(0.80±0.10)%比较,差异有显著性(P<0.01)。结论 CGRP释放增多可能参与气道慢性炎症的发病。
Changes ofcalcitonin gene-related peptide content in induced sputum from patients with COPD andasthma

【Abstract 】 Objective Toexplore the role of sensory neuropeptide calcitonin gene-related peptide (CGRP) in thepathogenesis of chronic airway inflammatory diseases COPD and bronchial asthma.MethodsPatients with COPD (n=19), bronchial asthma (n=14), all were in stable stage and 10normal volunteers were examined. After hypertonic saline inhalation challenge in allsubjects, CGRP-LI concentration in the induced sputum was measured by radioimmunoassay.Cellular content was assayed by microscopic analysis, the relation between CGRP-LI leveland FEV1 value was calculated by liner regression. Results The sputum CGRPconcentrations in patients with COPD and patients with asthma were (15.97±2.15)ng/L,(18.79±3.91)ng/L, respectively, both were significantly higher than those in normalvolunteers (2.36±0.35)ng/L. Moreover, CGRP concentrations in induced sputum in eachdisease group were correlated with the degree of airflow obstruction, r=-0.50 and -0.61,respectively (P<0.05). The percentage of neutrophil cell count (64.9±2.9)% wassignificantly higher in patients with COPD (P<0.01), while the percentage ofeosinophil cell count (5.8±0.5)% was increased in patients with asthma (P<0.01).Conclusions The data suggested that CGRP release may participate in the chronicinflammation of patients with COPD and bronchial asthma.
