【摘要】 目的 探讨系统性红斑狼疮(SLE)的发病机理和遗传基础。方法 分析了SLE患者中各种自身抗体与HLA-DR、DQ基因的关联。结果 发现抗Ro抗体与DR2、DQ6及DQ2/DQ6杂合子相关，抗RNP抗体与DQA1*0102、DQB1*0602相关。抗Sm和抗dsDNA抗体均与DRB1*1501、DQA1*0102、DQB1*0602相关，这与汉族SLE与HLA的关联完全一致，表明抗Sm和抗dsDNA抗体对SLE有极高的诊断价值。结论自身抗体与HLA的关联跨越了种族界限，不同种族的SLE患者中同种自身抗体与HLA抗原的关联基本相同，提示HLA对SLE的遗传易感作用主要发生在自身抗体水平。

    【Abstract 】 Objective To study the pathogenesis and genetic basis of systemic lupuserythematosus(SLE). Methods The association between HLA-DR,DQ genes and variousautoantibodies in patients with SLE was analysed. Results The results indicated that DR2,DQ6 and the heterozygous combination DQ2/DQ6 were closely correlated with theproduction of autoantibodies to Ro antigen,and DQA1*0102,DQB1*0602 with antibodyto RNP. As shown in the lupus population study in Han nationality,patients with anti-Sm andanti-dsDNA antibodies were associated with DRB1*1501,DQA1*0102,and DQB1*0602which suggests that anti-Sm and anti-dsDNA antibodies have a very important diagnosticvalue in SLE. Conclusion The association between autoantibodies and HLA has gone beyondethnic boundary. In various ethnic patients with SLE, the associaton between HLA antigenand specific autoantibody is similar. The above data suggest that contribution of class Ⅱ MHCmolecules in SLE is predominantly at the level of specific autoantibodies production.
