【摘要】 目的 制作犬马尾神经分级压迫动物模型,分析MRI与组织学改变间的关系,为临床治疗方法的选择、手术时机的掌握提供客观依据。方法 8只犬分为4组,每组2只,对照组仅行腰5~6椎板切除,其他3组行腰5~6椎板切除并且马尾神经分别被压迫25%、50%、75%。手术前后进行MRI检查,术后12周取材进行组织学分析。结果 对照组犬MRI及组织学检查未见异常。马尾受压25%的犬MRI示脑脊液信号消失,马尾神经信号稍高,组织学变化轻微。马尾受压50%的犬MRI示马尾神经有局部增高信号,组织学检查神经纤维粗细不等、有断裂及空泡形成。马尾受压75%的犬MRI可见马尾神经中断信号,组织学检查神经纤维部分消失。结论 MRI对马尾神经慢性压迫损伤可以作出早期提示,压迫超过50%将造成马尾神经不可逆损伤。
Comparative observation of MRI and histology of delayed graded compression of the dog cauda equina

【Abstract】 Objective To make an animal model of delayed graded compression of the dog cauda equina so as to analyze the relationship between the changes of MRI and histopathology, and to provide objective basis for the diagnosis and appropriate treatment. Methods There were four experimental groups, each containing two dogs. The control group had a laminectomy only. In the other three groups a laminectomy was performed and the cauda equina was constricted by 25%, 50%, and 75%, respectively. MRI was performed preoperatively and post-operatively. After three months of constriction, the cauda equina of the dogs was examined histologically. Results The dogs in the control group showed no changes in MRI and histopathology. The dogs with the cauda equina constricted by 25% had slight changes in MRI. In dogs with the cauda equina constricted by 50%, MRI appeared high signal intensity and the histological analysis of the cauda equina revealed nerve fiber degeneration. In dogs with the cauda equina constricted by 75%, MRI signal was suspended and the histology showed loss of fibers and wallerian degeneration. Conclusion MRI can make the early diagnosis of chronic compression of the dog cauda equina. Severe compression could cause irreversible damage to the cauda equina neurologically and histologically.
