磁共振脑血流灌注的影像及定量分析

【摘要】 目的 应用对比增强磁共振脑血流灌注成像,研究不同病理状态下脑组织的血液动力学变化。方法 采用对T2 * 敏感的平面回波自由衰减序列 (EPI-FID),对45例缺血性脑血管病、11例其他脑血管病、4例脑肿瘤和4例脑炎患者进行了注射对比剂后的血流灌注加权成像(PWI),分析脑血流灌注图,并定量地研究脑组织的血液动力学变化。 结果 磁共振脑血流灌注图和定量分析发现54个病例的脑组织有异常的血液动力学变化;PWI随访的全部急性脑梗死的病例均有缺血区血流灌注的恢复。结论 磁共振脑血流灌注成像可以定量地提供脑组织血流灌注变化的信息。

Qualitative andquantitative analysis of cerebral hemodynamics with MR perfusion-weighted imaging

Abstract】 Objective To study the changes of cerebralhemodynamics in different pathologic conditions with dynamic susceptibilitycontrast-enhanced MR perfusion-weighted imaging. Methods After bolus administration of Dimeglumine gadopentetate,rapid T2 * -weighted echo-planar free-induction-decay images wereacquired as perfusion-weighted images for multisections from 45 patients with ischemiccerebrovascular diseases, 11 with other cerebrovascular diseases, 4 with gliomas and 4with cerebritis. Cerebral blood perfusion maps were created and analyzed, as well absoluteregional cerebral blood volume, regional cerebral blood flow and mean transit time weredetermined and analyzed. Results In 54 cases, abnormal cerebral hemodynamics was observed.Follow-up examinations showed that in all acute cerebral infarction cases, there werereperfusion phenomena in ischemic regions. Conclusions MR perfusion-weighted imaging can provide quantitativeinformation of the changes of cerebral hemodynamics.
