环孢霉素A在青光眼滤过手术中抗瘢痕形成的临床应用

【摘要】 目的 通过在青光眼滤过术中应用2%环孢霉素A(CsA)与丝裂霉素(MMC)的对照研究，评价CsA用于青光眼滤过术的临床疗效及应用价值。方法 采取随机对照临床试验研究方法，将120例(180眼)难治性青光眼患者分甲、乙两组，每组60例(90眼)，分别在术中一次性应用2% CsA及0.2mg/ml MMC共5min，随访时间12～18个月。结果 (1)术后第18个月末，CsA组和MMC组功能性滤过泡的累积百分率分别为(81.25±10.13)%和(61.75±8.12)%，两组差异有显著性(P<0.05)。(2)CsA组和MMC组的累计完全成功率和条件成功率分别为(81.25±10.13)%、(92.62±3.38)%和(617±812)%、(79.37±6.12)%，差异有显著性(P<0.05)；(3)CsA应用于青光眼滤过术，其减少滤过手术失败的作用优于MMC，且并发症少。结论 青光眼滤过术中使用CsA可提高手术成功率。【关键词】 青光眼；滤过术；环孢素霉A；丝裂霉素C
A clinical study of CsA treatment in trabeculectomy

【Abstract】 Objective To compare the results of CsA treatment in trabeculectomy by a case-control study of CsA and MMC.Methods 60 cases (90 eyes) of intractable glaucomatous patients were randomly studied by treatment with CsA or MMC following trabeculectomy,each-group consisting of 90 eyes.2% CsA and 0.2mg/ml MMC were used for treatment of each group respectively for 5 minutes.The follow-up was ranged from 12 to 18 months.Results (1)The 18th month life-table success rate of functional bleb formation was (81.25±8.12)% in MMC group.The difference being statistically significant(P<0.05).(2)The 18th month life table complete and qualified success rates were (81.25±10.13)% and (92.12±3.38)% in CsA group,(61.7±8.12)% and (79.37±6.12)% in MMC group the difference being statistically significant (P<0.05).(3)Complications induced by CsA were more rare than MMC.No severe complications were found.Conclusion CsA has more better effect the MMC to reduce the risk of failure of glaucoma filtration surgery and no severe complications. ......
