【摘要】 目的 探讨腰椎管狭窄合并马尾神经损伤双侧半椎板开窗椎管扩大减压术的手术适应证及疗效和临床应用中的可行性。 方法 采用双侧经椎板间隙入路，腰椎管扩大减压并对神经根管扩大减压。治疗腰椎管狭窄，合并马尾神经损伤35例。 结果 随访1～3年，临床评价根据Nakei分级标准:优良30例，良5例，优良率86%。 结论 腰椎管狭窄合并马尾神经损伤双侧半椎板开窗腰椎管扩大术治疗。具有减压彻底，可同时行腰椎间盘摘除及神经根管松解减压，对腰椎稳定结构破坏小的特点，减少术后发生腰椎不稳及腰椎滑脱机率，能获得较好的治疗效果，适用于临床。
