摘要:目的 探讨变异型心绞痛发作时恶性心律失常乃至猝死的发生规律，为临床治疗提供依据。 方法 从因胸痛而行DCG检查患者中选择60例符合VA诊断标准的患者作为观察组，另选60例ST段压低的典型心绞痛患者作对照。比较分析观察组ST-T改变导联(即缺血部位)、形态、持续时间，以及发作时的心律失常、猝死前的心电图特点等。 结果 ①观察组的频发VE、尤其是恶性心律失常、窦性停搏发生率较对照组明显增多，两组比较，有非常显著性差异(P<0.05)。②观察组多部位病变者，其恶性心律失常发生率也明显增高(P<0.05)。③观察组VA发作时，ST段抬高≥4mm病例恶性心律失常发生率较ST段抬高<4mm病例明显增高(P<0.05);④观察组单向曲线及弓背向上型ST改变病例与上斜型病例比较，其恶性心律失常明显增多(P<0.05)。变异组在监测当天猝死者5例(8.33%)。 结论 VA发作时，ST段抬高≥4mm的单向曲线或弓背向上型改变者，尤其是近一周内VA频繁发作者是预后不良的指标，极易发生恶性心律失常以至猝死。强烈提示临床应对这类患者提高警惕，加强监测、给予及时的、积极的治疗。
