【摘要】 目的 研究原发性多脏器功能障碍综合征(multiple organ dysfunction syndrome,MODS)的整治与创伤评分的关系及器官功能障碍的发生频度和预后，为治疗提供依据。方法 回顾调查28例严重创伤后原发性MODS患者，入院时ISS评分值、脏器受累情况及治疗、预防情况，并做相关分析。结果 创伤ISS评分：20～30分8例，死亡2例；31～50分12例，死亡5例；>50分8例，死亡7例。在原发性MODS中，循环、呼吸功能障碍最常见，其次为中枢神经系统及血液系统。直接死亡原因依次为循环、中枢和呼吸功能衰竭等。结论 创伤评分直接反映原发性MODS的预后情况。治疗关键是治疗原发性创伤，包括“手术修复和脏器功能支持”。【关键词】 原发性多脏器功能障碍综合征；创伤评分
Clinical analysis for 28 cases of primary MODS caused by serious trauma

【Abstract】 Objective Research the relation between primary MODS and injury severity score and the occurrence frequentness and prognosis of MODS, and provide the basis for the treatment.Methods Review the 28 cases of primary MODS caused by serious trauma. Investigate their ISS and the circumstances of their organs injury, and know their treatment and prognosis, then analyze the above info.Results The ISS: In 20 to 30 points there are 8 cases, and 2 cases of them were death; In 31 to 50 points there are 12 cases, and 5 cases of them were death; over 50 points there 8 cases, and 7 cases of them were death. In the cases of primary MODS, the most was respiratory dysfunction and circulatory dysfunction, the next was central nervous system and hematic system injury. The order of the direct dead reason one by one was: circulatory dysfunction, central nervous system injury, respiratory dysfunction and so on.Conclusion The trauma score directly reflects the prognosis of primary MODS. The key of treatment is curing primary trauma including the surgical rehabilitation and the support of the organ function. ......
