摘要：目的：评价国产奥沙利铂(L-OHP，艾恒)联合氟尿嘧啶(5-FU)和亚叶酸钙(CF)方案治疗晚期胃肠癌的近期疗效和毒性反应。方法：27例晚期胃肠癌，其中胃癌9例，大肠癌18例，初治8例，复治19例，化疗前均经肘正中静脉留置中心静脉导管。给予FOLFOX方案，即L-OHP130mg/m2，静脉滴注4h，d1；CF 200mg/m2，静脉滴注2h，d1，5-FU 500mg于CF滴完后静脉推注，续以5-FU2.4～3.0g/m2持续静脉滴注48h，每3周重复。按WHO标准评价近期疗效和毒性反应。结果：全组患者获得PR12(51.9%)例，其中胃癌的有效率为44.4%；大肠癌的有效率为55.6%，初治有效率62.5%,复治有效率47.4%，统计学处理无明显差异(P>0.05)。主要毒副反应为恶心呕吐(74.0%)、口腔黏膜炎和腹泻(59.2%)，以及一过性感觉异常(55.6%)。结论：国产奥沙利铂联合CF/5-FU组成的FOLFOX方案治疗晚期胃肠道癌有效率较高，不良反应轻，值得进一步推广。

    The Clinical Observation of Advanced Gastric Cancer and Colon Cancer Patients Treated with Oxaliplatin,Calcium folinate and 5-Fluorouracil combination chemotherapy

    Abstract：Objective:To study the efficacy and toxicity of oxaliplatin(L-OHP)combined with calcium folinate(CF)and 5-fluorouracil(5-FU) in the treatment of advanced gastric/colon cancer.Methods:27 patients (9 cases gastric cancer,18 cases colon cancer) received chemotherapy of FOLFOX regimen:namely L-OHP130mg/m2 d1,continue injection (CI) for 4 hours;CF 200mg/m2 iv 2 hours d1,followed by 5-FU 500mg(bolus) and 5-FU 2.4-3.0g/m2 CI for 48hr with mini pump,q21d for 1 cycle.Results:The overall response rate was 51.9%,PR 14 cases,The response rate of gastric cancer and colon cancer was 44.4 % and 55.6% respectively.Significant difference was not observed between the two groups(p>0.05).The main side effects were nausea、vomiting(74.0%),mucositis and diarrhoea(59.2% ),The obvious side effect was sensory neuritis(55.6%),which was caused by L-OHP .Conclusion:L-OHP in combination with CF and 5-FU is an effective regimen in the treatment of advanced gastric/colon cancer,the toxicity is liable to be well tolerated. ......
