精氨酸局部治疗糖尿病足疗效观察

摘要:目的:研究局部应用精氨酸对糖尿病足治疗效果的影响。方法:27例糖尿病足患者随机分为2组:①常规治疗组:患者接受内科综合治疗和局部清创、抗生素、胰岛素治疗;②精氨酸治疗组:在常规治疗基础上局部加用精氨酸治疗。两组患者在治疗前后均检测踝动脉-肱动脉血压比值(ABI)和足趾皮肤温度。结果:精氨酸治疗组疗效优于常规组(P<0.05);精氨酸治疗组治疗后ABI指数明显增加,其治疗前后差值为0.11±0.05,而常规组为0.06±0.06(P<0.05);精氨酸治疗组治疗后足趾皮肤温度也明显增加,其治疗前后差值为1.32±0.68℃,而常规组为0.81±0.46℃(P<0.05)。结论:应用精氨酸局部治疗可以提高对糖尿病足的疗效,精氨酸能够促进糖尿病足溃疡创面愈合,改善足部血液循环。关键词:精氨酸;糖尿病足
Observation on Effect of Arginine Locally Administrated on Diabetic Feet

Abstract: Objective: To investigate the effect of arginine locally administrated on diabetic feet. Methods: 23 patients with diabetic feet were randomly divided into 2 groups:①routine-treated group: the patients received internal systematic treatment and debridement, antibiotics and insulin locally; ②arginine-treated group: the patients were rendered arginine locally on the basis of routine treatment. Both groups were measured ankle-brachial index(ABI) and the temperature of toe before and after treatment. Results: The therapeutic effects of arginine-treated group were better than those of routine-treated group(P<0.05);ABI of arginine-treated group had markedly increased after treatment and the difference value of ABI between antecedent and late treatment was 0.11±0.05, vs. routine-treated group 0.06±0.06(P<0.05); the temperature of toe of arginine-treated group had dramatically increased after treatment and the difference value of temperature between antecedent and late treatment was 1.32±0.68℃, vs. routine-treated group 0.81±0.46℃(P<0.05). Conclusion: Local administration of arginine can enhance the therapeutic effect on diabetic feet, and arginine can improve wound healing of diabetic feet ulcer and blood flow of feet. ......
