腹腔镜辅助下结直肠手术初探

【摘要】目的：探讨腹腔镜技术在结直肠手术中的应用价值。方法：腹腔镜辅助下行结直肠手术6例，其中乙状结肠癌根治术2例，直肠中上段癌行直肠前切除术(Dixon术式)3例，直肠下段癌行经腹会阴联合直肠切除术(Mile’s术式)1例。结果：4例手术获成功，2例中转开腹，无死亡病例，手术平均时间200min，术中平均失血60ml(40～100ml)，术后2～4d(平均24d)，肠蠕动恢复。结论：腹腔镜辅助下的结直肠手术具有患者创伤小，胃肠道功能干扰少，康复快等优点，安全可行。严格掌握手术适应证和充分准备术前各种仪器设备是完成此类手术的关键。【关键词】腹腔镜；结肠肿瘤；直肠肿瘤

【Abstract】Objective:To investigate the applicative value of laparoscope in colorectal operation.Methods:Six patients of colon or rectum procedure were performed,assisted by laparoscope,including two patients of sigmoid colon cancer radical resection,three patients of rectal anterior resection (Dixon) and one patient of abdominal perineal resection(Mile’s).Results:Four operations were successfully performed and two operations were converted to laparotomy.There was no death or serious complications.The mean operation time was 200min and average blood loss was 60ml.The enterocinesia recovered at mean 24 postoperative days.Conclusions:This surgical procedure has the advantages of less surgical trauma,less gastrointestinal interference and fast recovery.It can be carried out safely and feasibly further.At the same time,in order to smoothly accomplish the operations.It is crucial that the surgical indications should be strictly followed and the apparatuses should get good ready to the procedure. ......
