心室复极时程离散与缺血性心律失常的关系

关键词: 动作电位;心电描记术;QT间期;心律失常;心肌缺血摘 要:目的 观察兔急性心肌缺血前后心室复极时程的变化及其与缺血性心律失常的关系. 方法 结扎12只兔冠脉左室支(LVB)前后,分别测定其易损期(VVP)、LVB支配区及非支配区心外膜电图(EPG)的QT间期.并采用玻璃微电极技术记录动作电位(AP),测定APD90 、APD50 ,计算两区之间QT间期、APD90 、APD50 的差值,将缺血前后测算的上述各值作自身对照. 结果 结扎LVB前,两区QT间期、APD90 、APD50 相差较小,VVP不易测出,几乎为0;结扎后,缺血区的上述各值较结扎前明显减小,而非缺血区无明显变化,两区之间上述各值的差值较缺血前明显增加. 结论 急性心肌缺血后,心肌细胞的复极时程明显缩短,与非缺血部位的心室肌复极不同步,这是导致缺血性心律失常的重要原因.
