【摘要】 目的 观察胰岛素泵治疗2型糖尿病的疗效。方法 糖尿病患者32例随机分为两组，每组各16例，分别采用胰岛素泵持续皮下注射(continuous subcutaneous insulin infusion,CSII)和胰岛素多次皮下注射(many-times subcutaneous insulin infusion,MSII)治疗，根据血糖检测结果调整胰岛素用量。比较治疗前后血糖控制情况、胰岛素用量和低血糖发生率。结果 CSII组和MSII组相比较，空腹血糖水平治疗前差异无显著性(17.3±2.3mmol/L vs 15.1±2.1mmol/L,P>0.05)，治疗后均达满意控制(5.2±0.4mmol/L vs 6.6±0.3mmol/L,P>0.05)，而血糖达到良好控制水平所需时间CSII组明显缩短(4.2±1.4天 vs 12.6±3.2天，P<0.01)、胰岛素用量减少(43.4±13.8IU vs 54.4±18.3IU，P<0.01)。MSII组发生低血糖并发症6例，而CSII组则无低血糖发生。结论 CSII治疗2型糖尿病，可使血糖满意平稳控制，在血糖水平达到良好控制所需时间和胰岛素用量方面优于MSII常规治疗方法，并可减少或避免发生低血糖并发症，尤其适合反复发作低血糖或无症状低血糖的糖尿病患者。

    【Abstract】 Objective To observe of effect on type II diabetes mellitus with insulin pump treatment.Methods 32 consecutive patients with type II diabetes mellitus randomized to two groups:continuous subcutaneous insulin infusion (CSII) group and multi-time subcutaneous infusion (MSII) group,each one16 cases,respectively.Dosages of insulin used were on the basis levels of blood glucose measured.The level of blood glucose,developing the chance of low blood glucose were compared with in two groups before using the insulin and late.Results The level of blood glucose on an empty stomach were no difference in CSII group and MSII group before treatment with insulin (17.3±2.3mmol/L vs 15.1±2.1mmol/L,P>0.05) and come up to satisfied standards in trwo groups late (5.2±0.4;6.6±0.3,P>0.05).The spending time that blood glucose fall to achieve better standards in CSII group was shorter than MSII group (4.2±1.4 days vs 12.6±3.2 days,P<0.01).Total dosage of insulin was less in CSII group than MSII (43.4±13.8IU vs 54.4±18.3IU).There were complication of low blood glucose in 32 patients in MSSII group and no one in CSII group.The complication of low blood glucose in CSII group developed in 6 cases and no cases in MISS group.Conclusion It is a fine method for type II diabetes mellitus with insulin pump treatment in using total dosage of insulin,take time of fall blood glucose than MSII method able to reduce and the complication of low blood glucose and suitable specially for that patients occure low blood glucose repeatedly and no symptom. ......
