PTCA combined with stenting for chronic left heart

     Professor Jia Guoliang, chief of cardiology et al, Xijing Hospital, the fourth Military Medical University, adopted the technique of PTCA combined with stenting therapy for coronary heart disease with chronic left cardiac insufficiency had achieved remarkable long term satisfactory results with less complications. The use of PTCA combined with stent therapy for coronary heart disease with left cardiac insufficiency improved the safety of intervention treatment of coronary heart disease and ameliorated the long term prognosis. This is a technique not yet reported in the foreign literature. Professor Jia et al. started in 1990 the treatment of coronary heart disease with left cardiac insufficiency (LVEF≤40 %) and observed the effects during the acute period as well as the long term results. Up to December 1997, a total of 74 cases completed the treatment, among them 60 cases were treated with the support of the stent and in 30 cases the stent was implanted. The successful disease approaching rate was 89.5 % and the rate of successful treatment was 89.2 %. The rate of severe complication occurred during the peri-operation period was 2.7 %, among them there was a fatality rate of 1.4 %, myocardial infarction in 2.7 % and zero in coronary artery bypass graft (CABG). Successful treatment was obtained in 66 cases, with a follow-up period of 29±33 months (6-96 months); 58 cases survived, among them 44 cases had no cardiac events (cardiac death, myocardial infarction, PTCA, CABG, heart failure or recurrence of angina pectoris). The incidence of cardiac events in the stent group was 23.5 %, a figure lower than that in the simple PTCA group (43.8 %) (p=0.08). Among the 40 cases with symptoms of cardiac failure in the preoperative period, 29 cases experienced improvement of cardiac function 6 months after the operation.

    The results showed that the use of stent supported PTCA for the management of coronary heart disease with cardiac insufficiency resulted in a remarkable lowering of the incidence of severe complication during the acute period and a fatality rate remarkably lower than those of PTCA and CABG which were similar to those patients with normal cardiac function underwent intervention treatment. The long term survival rate of the successfully treated patients was higher that those underwent PTCA and CABG as was previously reported. The incidence of long term cardiac events in patients following stent implantation was lower than those underwent simple PTCA; following intervention treatment, the majority of the patients with heart failure enjoyed improvement of the cardiac function., http://www.100md.com
