Standardization of medication and dosage are equal
In a seminar on the new progress in the treatment of congestive heart failure convoked not long ago, Professor Wu Ning of Beijing PUMC Hospital recommended the result of a retrospective survey on the treatment of congestive heart failure (CHF) carried out in 11 hospitals in Shanghai.
During the three whole years i.e., 1980, 1990 and 2000, the most frequently used agents for the treatment of congestive heart failure in the above mentioned hospitals were diuretics (accounting for 77.69 %), then followed with nitrates (accounting for 75.26 % and digitalis (accounting for 60.43 %). The usage rate of angiotensin converting enzyme inhibitors (ACEI) increased rapidly, from 0.68 %, 38.81 % to 70.84 %. The use of β blockers for the treatment of (CHF) though remarkably increased in 2000 compared with that during the previous years, however, the usage rate was still below 25.02 % and the dosage given was inadequate.

Professor Wu Xuesi of Beijing Anzhen Hospital said that at the international seminar on congestive heart failure held in Norway June 2002, the American researchers announced the interim results of a large scale international study on carvedilol for the treatment of severe chronic congestive heart failure. The program began with a small dosage of 3.125 mg b.i.d., and continued with a gradually increasing dosage in every two weeks up to 25 mg qd.. After 8 weeks of treatment, the mortality decreased to 35 % (P=0.0013) in the carvedilol group compared with the controls and the rate of admissions also decreased in cases due to congestive heart failures and other causes. It was clearly seen that early, specified medication benefited patients with congestive heart failure earlier during the course of treatment.

Professor Yang Yuejin pointed out that some data showed that only less than one half of the clinical physicians were using ACEI and β blockers during the treatment of congestive heart failure, and they only prescribed 1/4 of the target dosage. As a result, not only the CHF could not be brought under control, also the medical resource was wasted. Therefore, the load is heavy while the way is long to educate the physicians carrying out specified management for CHF
