In the section of digestive disease, department of internal medicine, Beijing Tiantan Hospital, following clinical observation of using berberine hydrochloride for the treatment of irritable bowel syndrome (IBS), it was found that the drug was quite effective for remission of diarrhoea and the passing of mucous stool. This research topic found a new pathway of clinical application for a medical agent of the olden days.
A series of 99 cases of IBS who met Rome type II criteria, were randomized into the treatment, control and placebo groups of 33 cases each without significant difference in age, gender and course of illness among groups. In the treatment group 200 mg of berberine hydrochloride was given tid while in the controls, 100 mg of trimebutine (cerekinon, antagonist of calcium channel of gastrointestinal smooth muscle cell membrane) was administered tid. For the placebo group, 2 tablets of compound vitamin B2 was given tid. The treatment course of the three groups lasted for two weeks. The universal scoring criteria of IBS were used for the judgement of the effect.
The result revealed that the treatment group attained the highest effective rate as high as 70 %, 67.9 % in the controls; and in the placebo group the effective rate was only 37.5 %. Among them, the effect of berberine hydrochloride was most remarkable, giving an effective rate of 85.2%, not only far better than that in the placebo group, but also better than that in the cerekinon group. Berberine was also effective for mucous stool and abdominal pain giving an effective rate of 73.9 % and 78.6 % respectively. However, for patients with constipation, the effect was not ideal; therefore, for IBS patients with constipation the combined use of drugs for bowel motivation was advisable.
