兔骨髓基质干细胞体外培养复合β-磷酸三钙进行胸椎后侧融合实验研究

摘要:[目的]评价β-磷酸三钙复合骨髓基质干细胞为骨移植替代物进行脊柱融合的效果。[方法]60只兔子分四组进行胸椎后路融合:Ⅰ组(组织工程化骨即β-TCP复合BMSCs),Ⅱ组(β-TCP+自体红骨髓),Ⅲ组(单纯β-TCP),Ⅳ组(自体髂骨)。术后12周处死其余动物进行放射学、手工力学和组织学检查。[结果]从影像学和手工力学比较,Ⅰ组的融合率高于Ⅱ、Ⅲ、Ⅳ组且有统计学差异。组织学上,Ⅰ组新骨形成量较多,陶瓷微孔内部充满新生骨;Ⅱ组骨小梁形成量较Ⅰ组少, 为新生骨与纤维组织混杂;Ⅲ组陶瓷骨颗粒微孔内多为结缔组织,新生骨的量少,只见少量新生骨贴附于陶瓷骨表面; Ⅳ组在椎板与移植骨的交界处有稀疏的新生骨小梁形成。[结论]β-TCP复合BMSCs是脊柱融合自体骨的良好替代物。关键词:骨髓基质干细胞;β-磷酸三钙;脊柱融合;骨移植替代物
Bone marrow stromalderived osteoblastsporous βtricalcium phosphate composites as bone graft substitute for posterior thoracic spinal fusion

Abstract:[Objective]To evaluate the efficacy of autogenous bone marrow stromalderived osteoblastsporous(BMSCs) βtricalcium phosphate(βTCP) composites as an alternative to autogenous graft materials in spinal fusion.[Method]Posterior thoracic spine fusion was carried out in 60 White New Zealand rabbits using one of the following graft materials:βTCP granules plus cultured MSCs(group Ⅰ);β-TCP granules plus fresh autogenous bone marrow(group Ⅱ); β-TCP granules alone(group Ⅰ); and autogenous bone graft(group Ⅳ). All the rabbits were killed 12 weeks after surgery to evaluate with radiography, CT, manual palpation and histologic analysis.[Result]The rate of fusion was significantly higher in group Ⅰ compared with group Ⅱ, group Ⅲ and group Ⅳ; In group I histological analysis showed abundance newly formed bone in contact with the implanted granules;In group Ⅱ there were a reduced amount of newly formed bone and abundant fibrous tissue; In group Ⅲ there were relatively little new bone formation and abundant fibrous tissue; In group Ⅳ, there were thin trabeculae of newly formed bone closed to the decorticated lamina and some fibrous tissue.[Conclusion]The results indicate that bone marrow stromalderived osteoblastβ TCP composites may provide an well alternative to autogenous graft materials for spinal fusion. ......
