摘要:目的 研究选择性脑灌注最适宜温度范围。方法 应用选择性脑灌注模型对24只杂种狗在适宜灌注流量和灌注压下改变不同温度进行选择性脑灌注90min后, 通过上矢状窦(SSS)内不同阶段的静脉血乳酸测定、脑体感诱发电位(SEP)的连续监测及脑皮质组织病理学的检查来评估脑保护的情况。结果 在20~32℃范围内,未出现明显的脑功能及组织的不可逆损害。结论 选择性脑灌注在适宜的灌注流量及压力下,其温度维持在20~32℃对脑保护是安全有效的,在足够的脑灌流的情况下浅中低温可以明显地减轻对脑组织的损害。关键词:低温脑保护;体外循环;选择性脑灌注。
Experimental Study of Optimum Hypothermia for Selective Cerebral Perfusion

Abstract: OBJECTIVE Selective perfusion(SCP) for hypothermic cerebral protection in cardiopulmonary bypass .METHODS This study aimed at investigating the range of optimum hypothermia for cerebral protection by performing selective cerebral perfusion in 24 mongrel dogs for 90 minutes at different temperatures.RESULTS To evaluate the results of cerebral protection,venous blood lactate levels during different periods in superior sagittal simus (SSS) were measured,cerebral cortex histopathology was evaluated and somatosensory evoked potential(SEP) was monitored continuously.CONCLUSION Those results suggest that when selective cerebral perfusion was performed,with moderate flow rate and pressure,the safe and effective range of hypothemia for brain protection is from 20℃ to 32℃. Compared with deep hypothermia,moderate hypothermia can decrease the damage to brain obviously. ......
