【Abstract】 AIM: To develop a questionnaire for middle school students cognition examination. METHODS: Following open questionnaire answered by 54 middle school students and interview of 16 teachers, a preliminary questionnaire, composed of 65 items, was developed. The preliminary questionnaire was distributed to 720 middle school students, and the data of 687 (9542%) subjects were retrieved. The formal questionnaire was established by exploratory factor analysis and the reliability and the validity of the questionnaire were examined. RESULTS: The questionnaire consisted of 8 factors, namely: ability to prove oneself, negative automatic thoughts, over generalization, requital for elders, antipathy to test, test preparation, test selfefficiency, and test implication. The internal consistency reliabilities of the 8 factors were all more than 07, and the content validity and the structure validity of the questionnaire were good. CONCLUSION: The questionnaire has good reliability and the validity and can be used to examine middle school students cognition. ......
