(新疆医科大学第五附属医院骨科, 基础医学院病理学教研室, 新疆乌鲁木齐830011)摘要: 目的: 研究IL1β、TNFα、VEGF在不同类型腰椎间盘突出组织中的表达。方法:对70例腰椎间盘突出术后髓核组织,根据间盘突出类型分为膨出型组、突出型组、脱出型组,应用免疫组织化学方法,分别用IL1β、TNFα、VEGF单克隆抗体,对IL1β、TNFα、VEGF在不同类型的突出椎间盘中表达水平进行检测,并取7例尸体腰椎间盘标本设为正常对照。结果:腰椎间盘膨出型组IL1β、TNFα阳性表达分别为4例(31%)、3例(23%),VEGF无表达,IL1β、TNFα共同表达3例。腰椎间盘突出型组IL1β、TNFα阳性表达分别为27例(69.3%)、25例(64%),VEGF无表达,IL1β、TNFα共同表达21例。腰椎间盘脱出型组IL1β、TNFα、VEGF阳性表达分别为14例(89%)、13例(87%)、2例(12.5%),IL1β、TNFα共同表达11例。血管内皮细胞内可见VEGF表达,阳性率较低,7例正常腰椎间盘组织中IL1β阳性表达1例,TNFα阳性表达2例,VEGF无阳性表达。结论:在不同类型椎间盘突出中,IL1β、TNFα的阳性表达有显著差异。IL1β、TNFα、VEGF在腰椎间盘突出组织中的表达对临床腰椎间盘突出的治疗和转归预后有重要意义。

The expression of IL1β、TNFα、 VEGF in the herniated lumbar disc

Abstract: Objective: To study the expression of cytokines in tissues of different type of herniated lumbar disc. Methods: The tissues of herniated lumbar disc were obtained from 70 patients of lumbar intervertebral disc herniation who received surgery, then detected the expression level of IL1β, TNFα and VEGF in the different type of herniated lumbar disc through immunohistochemical method, and 7 normal specimen of lumbar discs were regard as a control group. Results: Positively expressing cells of IL1, TNFα were presented in exinflate lumbar disc were 4(31%), 3(23%) and no VEGF was found. In herniated lumbar disc, positive expressions of IL1, TNFα were 27(69.3%) , 25(64%) and no VEGF either. IL1β, TNFα and VEGF of protrusion lumbar disc were 14(89%) , 13(87%) and 2(12.5%). Positive expressions of VEGF were also found in endothelial cell, but the positive rate was low. As to the tissue of 7 normal lumbar disc, positively expressing cells of IL1β, TNFα, VEGF were presented in 1, 2 and 0 specimens. Conclusion: The positive expressions of IL1β, TNFα have significant difference in different type of herniated lumbar disc, so the expressions of IL1β, TNFα, VEGF take an importat role in therapy and prognosis of herniated lumbar disc. ......
