[摘要] 目的 探讨血清脑利尿钠肽(BNP)与早期糖尿病肾病(DN)的关系及其临床意义。方法 随机选择非肥胖2型糖尿病病人64例,按尿微量清蛋白排泄率(UAER)分为早期DN组(DN+组,UAER 20~200 μg/min,30例)和非DN组(DN-组,UAER<20 μg/min,34例);健康成人30例作为对照(CON)组。应用酶联免疫吸附法(ELISA)测定各组血清BNP水平。结果 DN-组血清BNP的水平明显高于CON组(F=6.82, q=4.30, P<0.01),DN+组血清BNP的水平高于DN-组(q=4.80, P<0.01);且与UAER呈明显的正相关(r=0.85,P<0.01)。结论 早期DN病人血清BNP水平显著升高, 可能在DN的发生、发展过程中起重要作用。[关键词] 脑利尿钠肽;糖尿病, 非胰岛素依赖型;糖尿病肾病;清蛋白尿
RELATIONSHIP BETWEEN SERUM BRAIN NATRIURETIC PEPTIDE AND EARLYSTAGE DIABETIC NEPHROPATHY

[ABSTRACT]ObjectiveTo investigate the relationship between the serum brain natriuretic peptide (BNP) and earlystage diabetic nephropathy, and to analyze its clinical significance. MethodsSixtyfour patients with nonobese type 2 diabetes mellitus and 30 normal controls were studied. According to urinary albumin excretion rate (UAER), the 64 patients were divided into two groups, one with microalbuminuria (DN+) and the other with normoalbuminuria (DN-). The serum levels of BNP were measured with competitive Enzyme Immunoassay kit.ResultsThe serum levels of BNP were significantly higher (F=6.82, q=4.30, P<0.01) in all the patients than those of normal subjects. The blood concentrations of BNP were significantly increased with microalbuminuria, compared with those with normoalbuminuria (F=6.82, q=4.80, P<0.01). The serum levels of BNP were positively and significantly correlated with UAER (r=0.85, P<0.01) in diabetic nephropathy.ConclusionThe serum levels of BNP were significantly increased in patients with early stage diabetic nephropathy, which probably play an important role in the occurrence and development of this disease. ......
