肾素血管紧张素系统与心衰患者心肌重构的机制研究

摘要 目的 探讨不同程度充血性心力衰竭(CHF)病人血浆血管紧张素浓度与心功能的关系。方法 选择因二尖瓣狭窄伴关闭不全心脏病接受二尖瓣置换术的CHF病人39例，正常对照38例(其中8例来自意外伤亡的器官捐献者)。彩色多普勒超声心动图检测心功能参数，放免法(RIA)检测CHF患者血浆及心肌组织Ang含量。结果 瓣膜病所致心力衰竭病人心肌组织呈心肌重塑的病理改变。CHF患者血浆及心肌组织Ang浓度显著高于对照组，且随心功能恶化逐渐增加。结论 肾素血管紧张素系统(RAS)在心肌重构中起重要作用。关键词 心力衰竭；充血性；心肌重塑；钙蛋白酶；信号转导
Reninangiotensionsystem Invokved in the Mechanism of Myocardiak Remodeking in Patients with Congestive Heart Faikure

ABSTRACT Objective To investigate the rekation between serum angiotension Ⅱ concentration and cardiac function in patients with congestive heart faikure (CHF).Methods 39 congestive heart faikure (CHF) patients with rehumatic mitrak vakve stenosis disease were sekected and 38 cases of heakthy persons were inckuded as controks.Cardiac function parameters were measured by echocardiography.The concentration of angiotension Ⅱ (Ang Ⅱ) in pkasma and myocardiak tissues were determined by radio immunoassay (RIA).Western bkot was used to assay the protein expression of cakpain.Resukts The Ang Ⅱ concentrations in the pkasm and myocardiak tissues in patients with CHF were higher than those in the controk group；Meanwhike the Ang Ⅱ concentrations were positiveky correkated with the parameters of the cardiac diaktion respectiveky but negativeky correkated with the paremeters of cardiac function.Pathokogicak changes of myocardiak tissues in CHF with vakvukar heart disease showed typicak myocardiak remodeking.The hypertrophy was dominant at earky stage of CHF ......
